Assessment of carotid artery intima media thicknes in patients with subclinical hypothyroidism

Öz:
Amaç: Karotis arter intima media kalınlığı sistemik aterosklerozun erken bir göstergesi olarak kabul edilmektedir. Bu çalışma, subklinik hipotiroidili hastalar ile sağlıklı kontrollerin ortalama karotis intima media kalınlığı (KİMK) de- ğeri açısından karşılaştırılarak; KİMK ile subklinik hipotiroidism arasında bir ilişki olup olmadığını tespitini amaçlamaktadır. Yöntem ve Gereçler: Subklinik hipotiroidili hastalar ile sağlıklı kontrollerin ortalama KİMK değerlerinin karşılaştırılmasını amaçlayan bu çalışmaya yeni tanı konmuş, ek kronik hastalık ve operasyon öyküsü olmayan 50 subklinik hipotiroidili hasta (ort(SS) yaş: 29.5(8.9) yıl) ve 50 sağlıklı ötiroid kontrol (ort(SS) yaş: 29.8(7.6) yıl) dahil edildi. KİMK ölçümleri Siemens Sonoline Ultrason cihazı ile B modunda olmak üzere aynı hekim tarafından yapıldı. Hastaların KİMK ölçümleri, her iki karotis arter bifurkasyonunun yaklaşık 10 mm. proksimali ile karotis arterin çıkış yerinden alınan ön ve arka duvar kalınlıklarının ortalaması alınarak hesaplandı. KİMK için bazal değer 0.4 mm olarak alındı. Subklinik hipotiroidili hastalar ile sağlıklı kontroller ortalama KİMK değerleri açısından karşılaştırıldı. Bulgular: Subklinik hipotirodili hastalar ve kontroller demografik özellikler, antropometirk ölçümler ve kan basıncı düzeyleri açısından benzerdi. Hastalarda daha yüksek olmak üzere TSH değerleri (7.1(2.2) ve 1.8(1.0), p=0.01) hasta ve kontrol grupları arasında anlamlı farklılık gösteren tek biyokimyasal değişken olarak saptandı. Ort(SS) KİMT değerleri subklinik hipotirodili hastalarda kontrollere göre belirgin olarak yüksek bulundu (0.4(0.2) ve 0.4(0.1), p=0.03). Subklinik hipotirodili hastalarda KİMT değerleri ile yaş (r=0.44, p=0.01), vücut kitle indeksi (r=0.34, p=0.024), sistolik (r=0.51, p=0.021) ve diyastolik (r=0.41, p=0.044) kan basıncı arasında anlamlı pozitif korelasyon bulundu. Sonuç: Sonuç olarak, bulgularımız subklinik hipotiroidili olgularda KİMT değerinde artışa ve dolaysıyla bu hastalarda artmış kardiyovasküler risk olasılığına işaret etmektedir.

Subklinik hipotiroidili hastalarda karotis intima media kalınlığının değerlendirilmesi

Öz:
Aim: Carotid artery intima media thickness (CIMT) is regarded as an early indicator of systemic atherosclerosis. The purpose of this study was to compare patients with subclinical hypothryoidism and euthyroid healthy controls in terms of average CIMT values to determine whether a relationship exists between CIMT and subclinical hypothy- roidism. Material And Methods: A total of 50 newly diagnosed subclinical hypothyroid patients (mean(SD) age: 29.5(8.9) years) without a history of operation and comorbid disease and 50 healty euthyroid control subjects (mean(SD) age: 29.8 (7.6) years) were included in this study aiming to compare CIMT values measured in patients with subclinical hypothryoidism and healthy controls. CIMT measurements was made by the same physician using Siemens Sonoline Ultrasound device in B mode, based on the average of CIMT measurements taken from 10 mm proximal of common carotid artery bifurcation bilaterally and anterior and posterior wall thicknesses taken from carotid artery outlet. Patients with subclinical hypothyroidism and euthyroid healthy controls were compared in terms of average CIMT values. Results: Subclinical hypothyroidism and control subjects were determined to be homogenous in terms of demographic characteristics, antropometric measurements and blood pressure levels. Overall, bichemical findings were similar between groups, except for the significantly higher mean(SD) serum TSH levels in patients vs. controls (7.1(2.2) vs. 1.8(1.0), p=0.01). Mean(SD) CIMT values were significantly higher among patients with subclinical hypothyroidism when compared to control subjects (0.4(0.2) vs. 0.4(0.1), p=0.03). In patients with subclinical hypothyroidism, significant positive correlations of CIMT values with age (r=0.44, p=0.01), body mass index (r=0.34, p=0.024), systolic (r=0.51, p=0.021), and diastolic (r=0.41, p=0.044), blood pressures were determined. Conclusion: In conclusion, our findings revealed that CIMT was increased in patients with subclinical hypothyroidism indicating the likelihood of increased risk of cardiovascular events in these patients.
