Emphysematous pyelonephritis (APN) is a rare, life-threatening, acute and severe necrotizing infection. Kidney tissue, collecting area or perinephritic tissues are usually affected in this disease. Gas presence is observed in the infection areas with the effect of gas-forming organisms. The disease may cause fever, pain in the renal area, acute kidney failure, septic shock or death. APN is a radiological diagnosis and computed tomography is the best imaging method for diagnosis. The disease is more common especially in the presence of conditions such as diabetes, alcoholism, immunosuppression and urinary tract obstruction. In APN case reports in the literature, it was reported that only one kidney, usually the left kidney, was affected and nephrectomy was performed in most patients as treatment. In this article, a case of bilateral APN presented with acute kidney injury and successfully treated with double j ureteral catheter and antibiotherapy is presented.
