Relations between Post-traumatic Stress Disorder, Dissociation and Attention-Deficit/Hyperactivity Disorder among Earthquake Survivors

Öz:
Amaç: Travma Sonrası Stres Bozukluğu (TSSB) ve dikkat eksikliği hiperaktivite bozukluğu (DEHB) arasındaki ilişkilere yönelik artan bir ilgi vardır. Pek az çalışma yapılmış olmasına karşın DEHB'nin TSSB ortaya çıkmasında bir risk faktörü olabileceğine ilişkin hipotezi destekleyen zayıf kanıtlar elde edilmiştir. Bunun yanı sıra TSSB ve DEHB arasındaki ilişkileri ele alan çalışmaların yetersizliği dikkat çekmektedir. Bu çalışmada TSSB ve DEHB arasındaki ilişkilerin disosiyatif patolojinin aracı etkisiyle birlikte ele alınması amaçlanmıştır. Yöntem: Araştırmaya çoğunluğu Van 2011 depremini yaşamış olan (%66) 317 lisans öğrencisi katılmıştır. Katılımcılara Travma Sonrası Tanı Ölçeği (TSTÖ), Disosiyatif Yaşantılar Ölçeği (DYÖ), Erişkin Dikkat Eksikliği ve Hiperaktivite Bozukluğu Kendi Bildirim Ölçeği (EDEHBÖ), Beck Depresyon Ölçeği (BDÖ) ve Beck Anksiyete Ölçeği (BAÖ) uygulanmıştır. Bulgular: Araştırmada DEHB belirtileri ve disosiyasyonun TSSB'yle ilişkisi manidar bulunmuştur. DEHB, TSSB ve disosiyasyon arasındaki çok değiş- kenli ilişkiler dikkate alındığında, TSSB ve DEHB arasındaki önemli ilişkilerin gerçekte belirti gruplarının benzerliğinden kaynaklandığı dikkat çekmektedir. Bununla birlikte, TSSB ve DEHB arasındaki ilişkilerde disosiyasyon aracı rol oynamıştır. Sonuç: DEHB komorbiditesinin travma sonrası stres cevabının gelişiminde baskın bir yatkınlık faktörü olmadığı, ancak TSSB geliştikten sonra belirtilerin kötüleşmesine neden olan bir etken olduğu sonucuna varılmıştır.

Depremden Sağ Kalanlarda Travma Sonrası Stres Bozukluğu (TSSB), Disosiyasyon ve Dikkat Eksikliği Hiperaktivite Bozukluğu (DEHB) Arasındaki İlişkiler

Öz:
Introduction: There is a burgeoning interest in relations between posttraumatic stress disorder (PTSD) and attention-deficit/hyperactivity disorder (ADHD). Although few studies were conducted, weak evidence was found supporting the hypothesis that ADHD may be a risk factor for the development of PTSD. In addition, there is a paucity of research addressing the relations between dissociation and ADHD. In this study, our aim was to examine the relations between PTSD and ADHD combined with the mediating effect of dissociative psychopathology. Methods: The participants were 317 undergraduate students, a greater proportion of whom experienced the 2011 Van earthquake (66%). The participants were administered the Posttraumatic Diagnostic Scale, Dissociative Experiences Scale, Adult ADHD Self-Report Scale, Beck Depression Inventory and Beck Anxiety Inventory. Results: We found that ADHD symptoms and dissociation were significantly associated with PTSD. Considering the multivariate relations between ADHD, PTSD and dissociation, significant associations between PTSD and ADHD resulted from symptom overlaps. However, pathological dissociation mediated the relations between PTSD and ADHD. Conclusion: We concluded that ADHD comorbidity was not a predominant vulnerability factor for the development of post-traumatic stress response but may be an exacerbating factor after the development of PTSD.
