Insula Volume in Patients Diagnosed with Obsessive Compulsive Disorder and its Relation with Clinical Variables

Introduction: Neuroimaging studies performed in recent years onpatients with obsessive compulsive disorder have determined anomaliesthat are possibly associated with the determinant signs and symptoms ofthe disease. The purpose of this study is to investigate the morphometricchanges in insula volume and related parameters in order to understandthe etiopathogenesis of obsessive compulsive disorder better.Method: Throughout the study, 20 patients eligible for the study criteria,who were diagnosed with OCD according to DSM-5 diagnostic criteriaand receiving inpatient or outpatient treatment after applying to FıratUniversity Hospital Psychiatry Department, and 13 healthy subjects inthe control group have been included in the study. Sociodemographicand Clinical Data Form, Hamilton Depression Rating Scale (HAM-D),Hamilton Anxiety Rating Scale (HAM-A), Yale-Brown ObsessionCompulsive Scale (Y-BOCS) and SCID-I have been applied to thepatients. Volumetric measurements were performed on the insula byusing magnetic resonance imaging (MRI) in patient and control groups.Results: No significant difference was observed between patients withObsessive Compulsive Disorder and healthy controls with regard toinsula volume.Discussion: It may be stated that insula may be associated with boththe pathophysiology and clinical course of the disease. It is suggestedthat studies considering the imaging methods examining the functionalcharacteristics of this area and tests on cognitive functions together maylead to significant and efficient results.

Obsesif Kompulsif Bozukluk Tanısı Olan Hastalarda İnsula Hacmi ve Klinik Değişkenlerle İlişkisi

Öz:
Amaç: Obsesif kompulsif bozukluğu olan hastalarda son yıllarda yapılan beyin görüntüleme çalışmaları, hastalığın belirleyici belirti ve bulgularıyla ilişkili anomalileri saptamaya yardımcı olmuştur. Bu çalışmanın amacı, obsesif kompulsif bozukluğun etyopatogenezini daha iyi anlamak için insula hacmindeki ve buna bağlı parametrelerdeki morfometrik değişiklikleri araştırmaktır. Yöntem: Çalışma süresince Fırat Üniversitesi Hastanesi Psikiyatri Anabilim Dalı’na başvuran ve yatarak ya da ayaktan tedavi gören, DSM-IV tanı ölçütlerine göre OKB tanısı almış ve çalışma ölçütlerine uyan 20 hasta ile 13 sağlıklı kontrol grubu çalışmaya dahil edilmiştir. Hastalara Sosyodemografik ve Klinik Veri Formu, Hamilton Depresyon Değerlendirme Ölçeği (HAM-D), Hamilton Anksiyete Değerlendirme Ölçeği (HAM-A), Yale-Brown Obsesyon Kompulsiyon Derecelendirme Ölçeği (Y-BOCS) ve SCID-I uygulanmıştır. Hasta ve kontrol grubunda manyetik rezonans görüntüleme (MRG) kullanılarak insulanın volümetrik ölçümleri gerçekleştirilmiştir. Bulgular: Obsesif Kompulsif Bozukluğu olan hastalar ile sağlıklı kontroller arasında insula volümleri açısından anlamlı düzeyde fark gözlenmemiştir. Sonuç: İnsulanın hem hastalığın patofizyolojisi hem de klinik seyri ile ilişkisinin olabileceği söylenebilir. Bu bölgenin fonksiyonel özelliklerini inceleyen görüntüleme teknikleri ve bilişsel işlevleri inceleyen testlerin birlikte ele alındığı çalışmaların daha önemli ve verimli sonuçlara ulaşılmasını sağlayabileceği düşünülmektedir.
