İnsomni yakınması ardında yatan uyku bozuklukları; huzursuz bacak sendromu ve uykuda periyodik hareket bozukluğu

Öz:
Huzursuz bacak sendromu (HBS) ekstremitelerde özellikle de bacaklarda tarifi mümkün olmayan anormal bir duyum, bir çeşit dizestezi ile karakterize bir hastalıktır. Bu anormal duyum / dizestezi karşı konulması mümkün olmayan hareket etme ihtiyacı ve motor huzursuzluğa neden olur. Uykuda periyodik hareket bozukluğu (UPHB) ise uyku sırasında periyodik olarak tekrarlayan, oldukça streotipik ayak, bacak ve/veya kol hareketleri ile şekillenen bir hastalıktır. HBS ve UPHB'nun sıklıkla birlikte görülüyor olması ve HBS ile ilişkili hareketlerin, uykunun başlaması ve derinleşmesi ile birlikte, UPHB'nun karakteristik özelliklerine sahip, daha yaygın ve düzenli bir hal aldığının gösterilmesi bu iki hastalığın, ortak bir santral sinir sistemi (SSS) bozukluğunun farklı iki klinik gösterimi olduğunu düşündürmektedir. Her iki hastalığın prevalansı da yaşla birlikte artış gösterir ve uykusuzluk yakınmasının en önemli nedenleridir. İdiyopatik olabilecekleri gibi diğer medikal, nörolojik veya primer bir uyku bozukluğuna eşlik edebilirler. Patofizyolojileri tam aydınlatılamamış olmakla birlikte, SSS'deki demir azlığının, uyku-uyanıklık durumuna bağlı olarak A11 dopaminerjik sistemde değişikliğe yol açtığı bunun da spinal eksitabilite artışına neden olduğu kabul edilmektedir Tedavide, başta dopamin agonistleri olmak üzere, L- Dopa, antiepileptikler, benzodiazepinler veya opioidler kullanılır.

Sleep disorders behind the complaint of insomnia; restless legs syndrome and periodic limb movements disorders of sleep

Öz:
Restless Legs Syndrome (RLS) is characterized by an abnormal sensation in the extremities, especially in the legs, which is a type of disesthesia difficult to define. This abnormal sensation/ disesthesia compels an urge to move in order to relieve the sensation and causes motor restlessness. Periodic Limb Movement Disorder of Sleep (PLMD) is a disease characterized by prominent periodically repeating stereotypic movements of the feet, legs and/or arms during sleep. Because RLS and PLMD often accompany each other and with the sleep onset and deepening of sleep, movements related to RLS turn into a more regular and extensive form characteristics of PLMD, one may think that these are two clinical variation of a common central nervous system disorder. Prevalance of both disorders increases with the age and one of the most important causes of insomnia. They may be idiopathic but may also be involved in other medical, neurological, or primary sleep disorders. Although pathophsiology of these disorders are not fully understood, it has been suggested that CNS iron deficieny which cause of the changes in A11 dopaminergic system in turn result in spinal excitability in a state dependent manner.In the treatment, preferably dopamine agonists, L-Dopa, antiepileptics, benzodiazepines and opioids are being used.
