Testis Kitlesi Serimiz; 150 Hastanın Retrospektif Analizi

Testis Kitlesi Serimiz; 150 Hastanın Retrospektif Analizi
Öz: Amaç: Testis tümörü nedeniyle radikal orşiektomi yaptığımız hastaların uzun dönem onkolojik sonuçlarını sunmayı amaçladık. Gereç ve Yöntem: Ocak 2010-Şubat 2022 tarihleri arasında kliniğimize testis tümörü öntanısı ile başvuran ve bu nedenle inguinal radikal orşiektomi operasyonu yapılan 150 hastanın verileri retrospektif olarak değerlendirildi. Patoloji sonucu germ hücreli testiküler tümör olarak gelen 141 hasta çalışmaya dahil edildi. Çalışmamızın birincil amacı uzun dönem sağ kalım sonuçlarını değerlendirmek idi. Bulgular: Hastalarımızın 8 (%5.7) inde kriptorşidizm öyküsü mevcut idi. Yetmiş yedi (%54.6) hastada sağ testiste tümör görülürken, 58(%41.1) hastada sol testiste, 6 (%4.3) hastada ise senkron veya metakron bilateral testis tümörü mevcuttu. Hastaların ameliyat öncesi ortanca AFP, ßHCG ve LDH değerleri sırasıyla 3.6 ng/ml, 1.7 mIU/ml ve 225 U/L idi. Ameliyat sonrası ortanca AFP ve ßHCG değerleri ise sırasıyla 3.2 ng/ml, 0.1 mIU/ml idi. Elli dört hastada (%38.2) lenf nodu tutulumu mevcuttu. Ortalama lenf nodu boyutu 2.32 cm idi. Hiçbir hastamızda visseral organ tutulumu yoktu. Hastaların ortalama takip süresi olan 76 ay süre zarfında testis kanseri nedeniyle sadece 1 (%0.7) hastada ölüm gerçekleşti. Hiçbir hastamızda TK haricinde herhangi bir sebepten ölüm olayı gözlenmedi. Sonuç: İleri evrede testis tümörü olan hastalarda dahi hem cerrahi hem medikal/radyasyon tedavisinin koordineli olarak planlanabilen merkezlerde sağ kalım oranlarının yüksek olduğunu belirtebiliriz.

Öz: Introduction: We aimed to present the long-term oncological results of patients who underwent radical orchiectomy for testicular tumor. Materials and Methods: The data of 150 patients who were admitted to our clinic with a preliminary diagnosis of testicular tumor and underwent inguinal radical orchiectomy between January 2010 and February 2022 were evaluated retrospectively. 141 patients whose pathology result was germ cell testicular tumor were included in the study. The primary aim of our study was to evaluate long-term survival outcomes. Results: Eight (5.7%) of our patients had a history of cryptorchidism. Seventy-seven (54.6%) patients had tumors in the right testis, 58 (41.1%) patients had left testis, and 6 (4.3%) patients had synchronous or metachronous bilateral testicular tumors. Preoperative median AFP, ßHCG and LDH values of the patients were 3.6 ng/ml, 1.7 mIU/ml and 225 U/L, respectively. Postoperative median AFP and ßHCG values were 3.2 ng/ml and 0.1 mIU/ml, respectively. Fifty-fourpatients (38.2%) had lymph node involvement. The mean lymph node size was 2.32 cm. None of ourpatients had visceral organ involvement. During the mean follow-up period of 76 months, only 1 (0.7%) patient died due to testicular cancer. No death was observed in any of our patients due to any reason other than TC. Conclusion:We can state that survival rates are high in centers where both surgical and medical/radiation therapy can be planned in coordination, even in patients with advanced testicular tumors.
