Sol Dal Bloğu İlişkili Kardiyomiyopati

Öz:
Sol dal bloğu (LBBB) ilişkili kardiyomiyopati (KMP) tanınırlığı giderek artan bir KMP’dir. Yeni kalp yetersizliği tanısı alan hastaların küçük ancak ihmal edilemeyecek bir kısmında bulunmaktadır. Ancak, halen dilate KMP veya sınıflanamayan KMP etiyolojik nedenleri arasında gösterilme- miştir. Tanı için farklı yayınlarda farklı kriterler önerilmiş ancak tanı kriterleri konusunda evrensel bir fikir birliği henüz oluşmamıştır. Bazı klinik özellikler ile elektrokardiyografi, ekokardiyografi ve kardiyak manyetik rezonans görüntüleme bulgularının, LBBB ilişkili KMP’nin tanısında kullanıl- ması önerilmektedir. Bu derlemede; LBBB ilişkili KMP prevalansı, patofizyolojik mekanizmalar, tanısal kriterler, tanı araçları ve spesifik bulgular diğer nedenlere bağlı dilate KMP ayırıcı tanısı da dikkate alınarak ele alınmaktadır.

Left Bundle Branch Block-Induced Cardiomyopathy

Öz:
Left bundle branch block-induced cardiomyopathy is an increasingly recognized type of dilated cardiomyopathy identified in a minority but not negligible proportion of patients with newly diagnosed heart failure. However, it has not yet been included among the possible etiologies of dilated cardiomyopathies or among the unclassified cardiomyopathies. Although a few sets of diagnostic criteria have been proposed, currently there is a lack of universal consensus regard- ing diagnostic criteria. Some specific clinical features and electrocardiography, echocardiogra- phy, and cardiac magnetic resonance imaging findings are recommended to help physicians in the diagnosis of left bundle branch block-induced cardiomyopathy. In this review, prevalence, pathophysiological mechanisms, diagnostic criteria, diagnostic modalities, and specific features of left bundle branch block-induced cardiomyopathy have been addressed with attention to the differential diagnosis of other dilated cardiomyopathies.
