Prognosis of lung cancer patients followed in the intensive care unit: A cross-sectional study

Introduction: Lung cancer is the most common solid organ malignancy requ- iring intensive care unit (ICU) admission. For many years, lung cancer patients were not considered in the priority patient category for admission to ICU because of their high mortality rate and poor response to therapy. Considering the developments in treatment modalities, we aimed to reevaluate the prog- nosis of patients with lung cancer in the ICU. Materials and Methods: Patients characteristics, date of diagnosis, the reason for ICU admission, the stage of cancer, histopathological type, history of che- motherapy, radiotherapy, or surgery for cancer, and APACHE-II and Charlson comorbidity index (CCI) were recorded retrospectively. Results: A total of 100 patients had a mean age of 69.7 ± 9.0 years. Among these patients, 18% had small cell lung cancer, while 82% had non-small cell lung cancer. The in-hospital mortality rate was 69% for all patients, while among those discharged from the ICU, the first 6-month mortality rate was 58.1%. The median survival time was 8.2 months. Advanced age, the need for mechanical ventilation, the need for vasopressors, a high APACHE -II, and the CCI all reduced survival in multivariate analysis, whereas chemotherapy and surgical history improved survival. Conclusion: Patients admitted to the ICU with lung cancer continue to expe- rience a high mortality rate. However, identifying the factors that are associa- ted with survival can be crucial in establishing care plans and prioritizing ICU admission for further therapy.

Öz:
Giriş: Akciğer kanseri, yoğun bakım ünitesine (YBÜ) yatış gerektiren en yaygın solid organ malignitesidir. Uzun yıllar boyunca akciğer kanseri hastaları, yük- sek ölüm oranları ve tedaviye zayıf yanıt vermeleri nedeniyle YBÜ'ye kabul için öncelikli hasta kategorisinde değerlendirilmemiştir. Tedavi modalitelerin- deki gelişmeleri göz önünde bulundurarak yoğun bakım ünitesinde yatan akciğer kanserli hastaların prognozunu yeniden değerlen- dirmeyi amaçladık. Materyal ve Metod: Hastaların özellikleri, tanı tarihleri, yoğun bakıma yatış nedenleri, kanserin evresi, histopatolojik tipi, kanser nede- niyle kemoterapi, radyoterapi veya cerrahi öyküsü, APACHE-II ve Charlson komorbidite indeksi (CCI) retrospektif olarak kaydedildi. Bulgular: Toplam 100 hastanın yaş ortalaması 69,7 ± 9,0 idi. Hastaların %18’inde küçük hücreli akciğer kanseri bulunurken, %82’sinde küçük hücreli dışı akciğer kanseri vardı. Hastane içi ölüm oranı hastaların %69’u iken, yoğun bakım ünitesinden taburcu olanlarda ilk altı aylık ölüm oranı %58,1 oldu. Ortalama hayatta kalma süresi 8,2 aydı. Çok değişkenli analizde ileri yaş, mekanik ventilasyon ihtiyacı, vazopresör ihtiyacı, yüksek APACHE-II ve CCI’nin tümü sağkalımı azaltırken, kemoterapi ve cerrahi öyküsü sağ- kalımı arttırdı. Sonuç: Akciğer kanseri nedeniyle yoğun bakıma kabul edilen hastalarda ölüm oranı hala yüksektir. Bununla birlikte, hayatta kalma ile ilişkili faktörlerin belirlenmesi, bakım planlarının oluşturulmasını ve ileri tedavi için yoğun bakım ünitesine yatışa öncelik verilmesini sağlayabilir.
