Non-Ketotik Hiperglisemi

Öz:
Non-ketotik hiperosmolar hiperglisemi, ketozis olmaksızın belirgin hiperglisemi ile karakterize, görme bo- zukluklarının yanı sıra letarji, konfüzyon ve komaya kadar varabilen nörolojik bulguların görülebildiği, yaşamı tehdit eden metabolik bir acil durumdur. Glukozüri ile birlikte şiddetli hiperglisemi, sırayla ozmotik diürezi ve progresif glial dehidrasyonu tetikler. Hızlı ve uygun tedavi edilmezse, serebral ödem, elektrolit düzensizliği, miyokard infarktüsü, inme, nöbetler ve santral pontin miyelinoliz gibi komplikasyonlara yol açabilir. Beyinde başlıca radyolojik bulgu manyetik rezonans görüntülemede T2 ağırlıklı serilerde oksipital bölgede subkortikal sinyal kaybıdır. Ayrıca difüzyon kısıtlılığı, perfüzyon değişiklikleri ve kortikal kontrast tutulumu gibi bulgular da görülebilir. Bu yazıda non-ketotik hipergliseminin klinik bulgular ışığında radyo- lojik görüntüleme bulguları sunulacaktır.
