Beliefs About Depression and Antidepressants: Is There A Relationship with Attachment Style or Personality Characteristics?

Öz:
Attitudinal barriers can be more frequent than structural barriers in depression treatment. Understanding attitudinal barriers are the first step to overcome. We aimed to investigate the relationship between the beliefs about depression, antidepressants with attachment styles, and personality characteristics. A total of 207 participants completed an online survey which consisted of the sociodemographic data form, beliefs about antidepressants and depression questionnaire, Experiences in Close Relationships-Revised, and Ten Item Personality Inventory. False beliefs about depression were related to younger age, lower education level, and male gender (p<0.05 each). False beliefs about antidepressants were related to openness to experience personality dimension and anxious attachment style in addition to the male gender and, lower educational level (p<0.05 each). Interventions addressing wrong beliefs about depression should provide basic information about the disease. Clinicians aiming to provide proper knowledge about antidepressants should prioritize a trusting relationship.

Classic and New Approaches to External and Middle Ear Malformations

Öz:
Abnormal development of the external ear causes the auricle deformity. A deformity in the external ear canal, middle ear, facial nerve and occasionaly in the inner ear may accompany the auricle deformity. Debate about classification of the ear anomalies still continue. Type I implies to the normal or near normal auricle in all classification systems. The severity of deformity increases in advanced types. Type III/IV imply ‘peanut’ like appearance auricle or anotia. On the other hand, if the auricle is normal in a case of congenital aural atresia, the middle and inner ear should be expected as normal with high possibility. The treatment of bilateral cases is quite known. The bone conducted hearing aids are used for bilateral cases in early term. The surgical intervention is performed in only obligatory situations (facial paralysis, cholesteatoma etc.) in unilateral cases. In other unilateral cases, the intervention can be postponed until adulthood. Physicologic problems may be the most important indication of the microtia repair. An auricle shuold be obtained for the use of both eyeglasses and hearing device besides satisfactory appearance. The hearing status impairs in atretic patients. Therefore, tympanoplasty should be performed to restore the hearing alongside aesthetic rehabilitation. Therefore, detailed evaluation of the patients for treatment options in early life period is very important. The options for auricle reconstruction and hearing rehabilitation should be examined meticulously for both unilateral and bilateral cases by taking into account of the patient’ status

Dış ve Orta Kulak Malformasyonlarına Yaklaşımda Klasik ve Yeni Uygulamalar

Öz:
Dış kulağın anormal embriyolojik gelişimi aurikulada deformiteyle sonuçlanır. Aurikula deformitesi olanlardasıklıkla dış kulak kanalı, orta kulak, fasiyal sinir ve düşük olasılıkta olsa iç kulakta da deformite olmasıbeklenmelidir. Kulak anomalileri sınıflaması ile ilgili tartışmalar halen süregelmektedir. Tüm sınıflama sistemlerindetip I normal ve ya normale yakın aurikulaya karşılık gelmektedir. Daha ileri tiplerde deformitenin şiddeti artmaktadır.Tip III/IV ise ‘yerfıstığı’ görünümlü kulak ya da kulak yokluğunu (anoti) ifade eder. Öte yandan bir konjenital auralatrezi olgusunda aurikula eğer normalse orta kulak ve iç kulak yapılarının normal olma olasılığı yüksektir. Tedavidebilateral aural atrezi olgularında yapılacaklar oldukça bellidir. Erken dönemde kemik iletimli işitme cihazlarıuygulanır. Unilateral olgulardaysa erken dönemde cerrahi müdahale, sadece gerekli durumlarda (kolesteatoma,fasiyal sinir paralizisi gibi) yapılır. Diğer unilateral olgularda hasta ve ailesi karar verene dek erişkin çağa kadarbeklenebilmektedir. Mikroti onarımının belki de en önemli endikasyonu anormal görüntünün neden olduğu psikolojiksonuçlardır. Tatminkar görünümün yanısıra gözlük ve işitme cihazı kullanılmasına uygun aurikula elde edilmelidir.Atrezili hastalarda işitme de bozulmuştur. Estetik rehabilitasyonun yanı sıra işitmeyi düzeltmeye yöneliktimpanoplasti prosedürleri de yapılmalıdır. Bu nedenle hastaların tedavi seçenekleri bakımından yaşamın erkendöneminde uygun olarak değerlendirilmesi önemlidir. Aurikula rekonstrüksiyonu ve işitme rehabilitasyonuseçenekleri unilateral ve bilateral olgularda hastanın durumu da göz önüne alınmak suretiyle titizlikle ele alınmalıdır.

Antidepresanlar ile İlişkili Mitlerin Bağlanma Stilleri ve Kişilik Özellikleri ile İlişkisi

Öz:
Depresyon tedavisinde tutum ilişkili engeller yapısal olanlardan daha sık olabilir. Tutum ilişkili engelleri anlamak üstesinden gelmenin ilk adımıdır. Bu çalışma depresyon ve antidepresanlar ile ilgili inançların bağlanma biçimleri ve kişilik özellikleri arasındaki ilişkiyi araştırmayı amaçlamaktadır. Sosyodemografik veri formu, antidepresanlar hakkındaki inançlar ve depresyon anketi, Yakın İlişkilerde Yaşantılar Envanterir-Gözden Geçirilmiş Formu ve On Maddelik Kişilik Envanteri'nden oluşan çevrimiçi anketi toplam 207 katılımcı doldurdu. Depresyonla ilgili yanlış inanışlar, genç yaş, düşük eğitim düzeyi ve erkek cinsiyet ile ilişkiliydi (her biri için p<0,05). Antidepresanlarla ilgili yanlış inanışlar, deneyime açıklık kişilik boyutu, kaygılı bağlanma biçimi, erkek cinsiyet ve düşük eğitim düzeyi ile ilişkili bulunmuştur (her biri için p<0.05). Depresyonla ilgili yanlış inançları ele alan müdahaleler, hastalık hakkında temel bilgiler sağlamalıdır. Antidepresanlar hakkında doğru bilgi vermeyi amaçlayan klinisyenler, bir güven ilişkisine öncelik vermelidir.
