Aleksitimi: Psikolojik bir semptom mu, yoksa bir kişilik özelliği mi?

Öz:
Yunanca bir kavram olan aleksitimi, genel olarak duygularını tanıma ve sözel olarak ifade etmekte güçlük çeken bireyleri tanımlamak amacıyla kullanılmıştır. Araştırmalar ve klinik gözlemler, ilk olarak psikosomatik hastalıklarda görülen aleksitiminin, depresyon, travma sonrası stres bozukluğu, madde kötüye kullanımı ve madde bağımlılığı gibi pek çok psikolojik bozuklukta da gözlendiğini ortaya koymaktadır. Buna karşın, son yıllardaki araştırmalar, klinik durumların yanı sıra, aleksitiminin normal populasyonda da gözlenen bir olgu olduğunu göstermektedir. Bu durum, aleksitiminin kişisel bir eğilim ya da bir kişilik özelliği mi, yoksa psikolojik bir bozukluk veya bir semptom mu olduğu konusundaki görüş ayrılıklarını daha da belirgin hale getirmektedir. Daha da ötesinde, aleksitiminin kalıcı mı, yoksa geçici mi olduğu konusunda da anlaşmazlıklar vardır. Bu inceleme makalesinin amacı, tarihsel bir perspektif içerisinde aleksitimi kavramım ele almak ve literatür ışığında aleksitminin klinik bir kavram mı, yoksa kişisel bir eğilimi yansıtan genel bir fenomen mi olduğu sorusuna yanıt aramaktır. Bu genel amaç çerçevesinde öncelikle, aleksitiminin etiyolojisini açıklayan teorik modeller gözden geçirilecek, sonrasında ise aleksitimi kavramının doğasına ilişkin çalışmalara yer verilecektir.

Alexithymia: A psychological symptom or a personality characteristic?

Öz:
Alexithymia is a Greek term which is used to describe individuals who have difficulty to define and verbalize their emotions. Studies and clinical observations have shown that alexithymia which was first observed in psychosomatic patients has also been seen on several psychological disorders such as depression, post-traumatic stress disorder, substance abuse and dependence. Nevertheless, recent research indicates that alexithymia is a phenomenon which is also seen in normal population besides clinical context. This situation leads us to ask the question whether alexithymia is a psychological disorder, a symptom, an individual tendency or a personality characteristic. Furthermore, there are some disagreements around whether alexithymia is reversible or irreversible. The purpose of this review is to examine the alexithymia concept within its historical perspective and to search for an answer to the question whether alexithymia is a clinical concept or a general phenomenon reflecting a normal individual tendency. Within this general framework, firstly, the etiological models of alexithymia will be reviewed, and then the studies which explain the nature of alexithymia will be examined.
