Diş hekimliğinde obez hastalara yaklaşım

Öz:
Obezite ve aşırı kilo vücut sağlığını bozacak ölçüde yağ dokusundaki anormal veya aşırı yağ birikmesi olarak tanımlanmaktadır. Obezitenin ve aşırı kilonun temel nedeni, diyetle alınan enerji miktarı ile metabolizma ve fiziksel aktiviteler sırasında harcanan enerji miktarı arasındaki düzensizliktir. Aşırı kilo ve obezite prevalansı dünyanın çeşitli bölgelerindeki gelişmiş ve gelişmekte olan ülkelerde ciddiyetle dikkate alınması gereken oranlara ulaşmıştır. Obezite başta Tip 2 diyabet, hipertansiyon ve kardiyovasküler sistem hastalıkları olmak üzere pek çok kronik enflamatuar hastalık ve kanser gelişimi için temel bir risk faktörü olarak kabul edilmektedir. Son dönemdeki çalışmalar obezitenin ağız hastalıkları, özelliklede periodontitis ile ilişkili olduğunu göstermiştir. Bunun yanı sıra çürük, diş kaybı, travmatik diş yaralanmaları, ağız kuruluğu ile de ilişkilidir. Obezite prevalansının dünya çapında bir artış eğiliminde olması, diş hekimi kliniklerinde obez hastalarla çok daha sık karşılaşılması ihtimalini artırmaktadır. Kliniğe ulaşım imkânları ve tedavi ortamının ergonomisi obez hastalar için bazı modifikasyonlar gerektirebilir. Hastanın muayenesi, tedavi için hazırlanması, konservatif veya cerrahi müdahaleler ve ilaç tedavisi aşamaları obez hastalarda bazı spesifik yaklaşımları gerektirmektedir. Bu derlemede diş hekiminin obez hastalara yaklaşımına yardımcı olabilecek bilgilerin güncel literatürler ışığında değerlendirilmesi amaçlanmaktadır.

The approaching the obese patients in dentistry

Öz:
Obesity and overweight are defined as abnormal or excessive fat accumulation that may impair health. The basic cause of obesity and overweight is the disarrangement between the energy spent on physical activities and metabolism and quantity of the energy gained by the way of nutrition. Overweight and obesity prevalance reached to significant rates in developing and developed countries on various regions of the World. Obesity is regarded as a basic risk factor especially for Type 2 Diabetes, Hypertension and cardiovascular system diseases and many chronic inflammatory diseases and evolution of cancer. The latest studies demonstrated that obesity is related to oral diseases and especially periodontitis. It is also related to dental caries, loss of tooth, traumatic injuries on teeth and xerostomia. The worldwide increase of obesity prevalance increases the possibility of encountering with obese people in clinics of dentists. Transportation opportunities of clinic and ergonomics of the treatment environment may require some modifications for obese people. Stages of examination of the patient, preparation for treatment, conservative or surgical interventions and drug treatment require some specific approaches on obese people. On this eclectic study, the goal is the evaluation of helpful information on approach of dentists to obese people under the illustration of actual literatures.
