Hipotiroidili gebede anestezik yaklaşım

Öz:

Hipotiroidizm, genellikle artmış tiroid stimulan hormon (TSH) değeri ile subklinik formdan miksödeme kadar değişiklik gösteren bir tablodur. 34 yaşında, 70 kg, 160 cm boyunda ve 40. gebelik haftasında olan takipsiz hastanın hipotiroidi nedeniyle düzensiz Levotiroksin sodyum ® (Levotiron 0,1 , Abdi İbrahim/İstanbul) kullanım hikayesi mevcut. Yapılan laboratuar tetkiklerinde sT3: 2.49 pq/ml, sT4: 0.85 ng/ml iken TSH değeri 26.61 U/ml olarak yüksek bulundu (TSH: 0.27-4.20). Bu hastalarda anestezi duyarlılığı artmış olduğundan miyokard fonksiyonunda depresyon, hipoksik ve hiperkapneik solunum cevabında azalma, anormal baroreseptör fonksiyonu, plasma volümünde azalma ve karaciğer ilaç metabolizmasında bozulma görülebilir. Acil operasyonlar dışında cerrahi öncesi mutlaka ötiroid hale getirilmelidir. Pratik uygulamamızda, fetal distres nedeniyle acil operasyona alınan takipsiz hipotroidili gebede rejyonel anestezi uygulamamızı literatür eşliğinde sunmayı amaçladık.

Anesthetic management in pregnancy with hypothyroidism

Öz:

Hypothyroidism is usually elevated thyroid-stimulating hormone (TSH) that is changed from subclinic to myxedema form. 34 years old pregnant woman who is 70 kg weight, 160 cm height and 40 weeks gestation. In her medical history hypothyroidism has been diagnosed for five years and has irregular Levotiroksin sodium using. The laboratory tests values fT3: 2.49 pg/ml, fT4: 0.85 ng/ml while the TSH 26.61 U/ml were found (TSH: 0.27 to 4.20). These patients have increased sensitivity to anesthetic depression of myocardial function, the respiratory response to hypoxic and hipercapneic reduction, abnormal baroreceptor function, reduction in plasma volume and hepatic drug metabolism may be impaired. Except for emergency operations should be euthyroid before surgery necessarily. In our practice, we aimed to report regional anesthesia management for untreated hypothyroid pregnant woman undergoing emergency surgery in the light of the literature.
