Antibiotic susceptibility of bacteria ısolated from urinary cultures taken from children

Öz:
Amaç: Bu çalışmada hastanemizde 2009-2011 yıllarıarasında, çocukluk çağı idrar kültürlerinden izole edi- len bakterilerin antibiyotik duyarlılıklarını belirlemeyiamaçladık. Her hastanenin kendi direnç profilini çıkarması ile klinisyenlerin idrar yolu enfeksiyonların- da ampirik antibiyotik seçiminde daha doğru kararlar verebileceğini düşündük. Ayrıca kısıtlı antibiyogram bildirimi ve akılcı antibiyotik kullanımı ile ilgili verilen eğitimler sonrasında direnç oranlarındaki değişimleri gözlemledik. Gereç ve Yöntemler: Bu amaçla 2009-2011 yılları arasında istenen 19138 adet idrar kültürü retrospektif olarak incelendi. Örnekler kanlı agar (premed, İstanbul-Türkiye) ve EMB (premed, İstanbul-Türkiye) besiyerlerine ekildi. Duyarlılık testleri NCCLS kriterleri doğrultusunda disk difüzyon yöntemiyle araştırıldı.Bulgular: İdrar kültürlerinde 2009 yılında 490 (%10.2)hastada, 2010 yılında 377 (%5.6) hastada, 2011 yılın-da ise 905 (%11.7) hastada üreme saptandı. En sık üreyen bakteri her üç yıl için de Escherichia coli’dir.Ampisilin, ko-trimaksazol, ampisilin sulbaktam ve amoksisilin klavulonik asit ampirik kullanımda dikkat edilmesi gereken ve yüksek direnç oranları göz önün-de bulundurulması gereken antibiyotiklerdir. Birinci veikinci kuşak sefalosporinler düşük direnç oranlarınedeniyle ampirik tedavide kullanılabilecek antibiyo-tiklerdir.Sonuç: Antibiyotik direnç oranları bölgeden bölgeyegösterdiği için klinisyenler idrar yolu enfeksiyonların- da ampirik antibiyotik yazarken o bölgenin verilerine göre seçim yaparlarsa tedavilerde başarı şansı dahayüksek olacaktır. Ayrıca akılcı antibiyotik kullanımı ile ilgili verilen eğitimler ve kısıtlı antibiyogram bildirimisonrasında direnç oranlarındaki düşüş umut vericidir. Eğitimlerin her yıl periyodik olarak tekrarlanması fay-dalı olacaktır. (J Pediatr Inf 2013; 7: 7-12)

Objective: İn this study we aimed to determine anti-biotic susceptibility of bacteria isolated from child-hood urinary cultures in our hospital between 2009-2011.We thought that clinicians would make betterdecisions in selecting empirical antibiotics in urinarytract infections with every hospital publishing its ownresistance profile. In addition, we observed changesin resistance ratios after training in rationalistic antibi-otic useage and limited antibiogram notifications.Material and Methods: For this purpose, 19138 uri-nary cultures taken between 2009-2011 were anal-ysed, retrospectively. The samples were implantedinto blood agar (Premed, Türkiye) and EMB agar (Premed, Türkiye). Sensitivity tests were researched with the disk diffusion method in accordance with NCCLS criteria. Results: Reproduction was detected in 490 patients (10.2%) in 2009, in 377 patients (5.6%) in 2010, and in 905 patients (11.7%) in 2011. The most repro- duced bacteria was Escherichia coli in all three years. Ampicilline, co-trimaksazole, ampicilline sulbactam and amoxicillin-klavunat were the antibiotics which have to be given attention in using empirical usage and have to be taken into account for their high resis- tance ratios. First and second generation cephalo- sporins are the antibiotics that could be used for the empirical treatments due to their low level of resis- tance ratios. Conclusion: Due to the fact that antibiotic resistance ratios vary from region to region, clinicians would have more chance in treatment if they take into con- sideration the selective region’s data in writing empir-ical antibiotics in treatment of urinary tract infections.Besides,decline in resistance ratios after training inrationalistic antibiotic useage and limited antibiogramnotifications is hopeful. The repetition of trainingcourses periodically each year will be useful.(J Pediatr Inf 2013; 7: 7-12)
