Central venous catheter-related bloodstream infections: literature review

Öz:
Santral venöz kateter (SVK) kullanımı düşük doğumağırlıklı yenidoğanlar, kanserli çocuklar ve kistik fibro- zis ve hemofili hastaları gibi sık damar yolu ihtiyacıolan kronik hastalığı olan çoğu çocuk hasta grubundaoldukça faydalıdır. Santral venöz kateterler genellikleiyi tolere edilmekle beraber enfeksiyöz komplikasyon-lar olabilmekte ve artan morbidite, hospitalizasyon ve maliyete neden olabilmektedir. Çocuklarda vaskülererişim sıklıkla çok sınırlıdır. Bu durum periferal kan kültürü elde etmenin zorluğu ve santral yolun uzaklaş- tırılmasının işlevsel olmaması nedeniyle kateter-ilişkilikan dolaşım enfeksiyonlarının tanı ve tedavisini güç- leştirmektedir. Veriler kısıtlı olmakla beraber, santral venöz kateter-ilişkili enfeksiyonlarından (SVKE) korun-ma ve tedavisi ile ilgili yeni teknolojiler SVKE’lerinazalmasında etkili olabilmektedirler. Santral venöz kateter-ilişkili enfeksiyonların patogenezini ve altta yatan risk faktörlerini anlamak hayatı tehdit edicikompliksayonları azaltmak ve yeni tedavi stratejilerigeliştirmek açısından çok önemlidir.(J Pediatr Inf 2013; 7: 25-30)

Öz:
The use of central venous catheters (CVC) is benefi-cial for the management of many groups of pediatricpatients including low birth weight neonates, childrenwith cancer, and those with chronic diseases thatrequire frequent venous access such as cystic fibro-sis and hemophilia. These CVCs are usually well-tol-erated but infectious complications are relativelycommon and result in increased morbidity, hospital-ization, and cost. Vascular access is often limited inpediatric patients. This affects both the diagnosis andmanagement of catheter-related bloodstream infec-tions (CR-BSIs) because obtaining a peripheral cul-ture may not be possible and line removal may not befeasible. Despite limited data for specific interven-tions, new technologies for the prevention and treat-ment of CR-BSIs are effective in reducing CR-BSIs.Understanding of the pathogenic process and under-lying risk factors of CR-BSIs is important in decreas-ing the life threatening complications and alsoimproving the management strategies.(J Pediatr Inf 2013; 7: 25-30)
