Gebelik İle İlişkili Osteoporoz

Öz:
Osteoporoz, düşük kemik kütlesi ve kemik dokusunun mikromimarisindebozulma ile karakterize sistemik bir iskelet hastalığıdır. Osteoporozunsık görülmesinin aksine gebelikte görülen osteoporoz, etiyolojisi vepatofizyolojisi iyi bilinmeyen nadir bir klinik sorundur. Gebeliğin bir sonucuolarak mı olduğu yoksa rastlantısal olarak mı gebelikle birlikte görüldüğühalen yanıtlanması gereken sorulardır. Gebelik ve laktasyon dönemindekemik ve mineral metabolizması ile ilişkili değişiklikler fetüse yeterlikalsiyum geçişine izin verir. Bu nedenle gebelik ve emzirmenin kemikkaybı üzerinde önemli bir etkisi bulunmakta olup bu durum gebeliğe bağlıosteoporoz ve ilişkili kırıkların nedeni olabilir. Belirtiler sıklıkla ilk gebeliksırasında oluşur ve genellikle tekrarlayıcı değildir. Ayrıca ikincil osteoporoznedenleri de, gebelik sırasında ağrı ve kırıklara neden olabileceği için gözönünde bulundurulmalıdır. Şu ana kadar gebelik ile ilişkili osteoporoztedavisi ile ilgili herhangi bir kılavuz bulunmadığı gibi herhangi bir ortakgörüş de sağlanabilmiş değildir. Bu nedenle tedavi planına karar verirkenher hastanın bireysel olarak değerlendirilmesi ve tedavinin buna göreplanlanması gerekir. Türk Fiz T p Re­hab Derg 2013;59:145-50.

Pregnancy Associated Osteoporosis

Öz:
Osteoporosis is a systemic skeletal disease characterized by low bone massand microarchitectural deterioration of bone tissue. While osteoporosis isseen very often, osteoporosis in pregnancy is a rare clinical problem withunknown etiology and pathophysiology. Whether pregnancy is accidentalor is a direct cause for osteoporosis are the questions that remainunanswered. The normal pregnancy and lactation-induced changes inbone and mineral metabolism allow adequate delivery of calcium to thefetus, mostly in the last trimester of pregnancy. Therefore, pregnancy andlactation may have significant impact on bone loss and this may result inpregnancy-induced osteoporosis and the related fractures. The symptomsoccur often during the first pregnancy and usually does not recur.Secondary causes of osteoporosis that may cause back pain and fracturesduring pregnancy should also be taken into consideration. Recently, noguidelines exist about the therapy of pregnancy associated osteoporosis,this means each case must be evaluated on an individual basis in order todecide on a treatment plan.
