Öz:

Güçlü bir birinci basamak (BB), bir ülkenin tüm gereksinimlere yanıtveren, yüksek nitelikli ve maliyet etkili bir sağlık sistemine ulaşma kapasitesine katkıda bulunacaktır. Şu anda var olan ve BB sağlık hizmetlerini önceleyen politikalara temel oluşturan kanıtlar, dar coğrafyalarda yapılmış, sınırlı ölçütlerin kullanıldığı ve görece eski verileredayanan çalışmalardan gelmektedir. Hollandada Sağlık SistemleriAraştırmacısı olan Dionne Sofia Kringosun yakın zamanda yayınlanmış doktora tezi, Avrupada BB'yi (ve onun gücünü) oluşturan bileşenleri ve bunların sağlık sistemi çıktıları üzerindeki etkisini incelemeyi amaçlamaktadır. Bu makalede farklı yerlerde yayınlanmış söz konusu çalışmanın sonuçları özetlenmektedir. Birinci basamağın yapısalgücü, bu yapının üç boyutu ile ölçülmüştür: Yönetim, BB işgücününgelişmişliği ve ekonomik koşullar. Birinci basamak sağlık hizmetlerisunumu sürecinin gücü ise dört boyutta değerlendirilmiştir: Erişilebilirlik, süreklilik, eşgüdüm ve kapsamlılık. Bu boyutlar, 31 Avrupa ülkesinden toplanan verilerde toplamda 77 gösterge kullanılarak değerlendirilmiştir. Sonuçlar Avrupa ülkelerinin birinci basamağın gücüaçısından farklılıklar gösterdiğini ortaya koymakta ve ulusal veyauluslararası politika yapıcıların birinci basamağa yükledikleri sorumluluk ve örneğin gelecekteki işgücü açığının çözümü için birinci basamakta gereksinim duyulan yatırım bakımından ayrışmalara işaret etmektedir. Ülkeler, tüm önemli yapısal boyutlar açısından tutarlı birşekilde BB odaklıdırlar ve daha iyi bir performans yönetimi sağlayabilmek için BB bilgi altyapılarını geliştirmelidirler. Bu çalışma, güçlü birbirinci basamağın toplum sağlığı üzerindeki olumlu etkisini, eşitsizliklerin azaltılmasında ve gereksiz hastane yatışlarının önlenmesindekikatkısını göstermektedir.

Öz:

Strong primary care (PC) is supposed to improve the capacity of acountry to achieve a responsive, high quality and cost-effectivehealth care system. The available evidence-base supporting pro-pri-mary carepolicies originates from studies with a limited geograph-ical scope, narrow use of dimensions to measure PC strength, anduse relatively old data. The recently published PhD Thesis by DionneSofia Kringos (Health Systems Researcher from The Netherlands)aimed to get insight into the elements that form (the strength of)primary care in Europe and their impact on health care system out-comes. This article summarizes the results of this work, which hasbeen published in more detail in different scientific publications.The strength of PC was measured by 3 dimensions of PC structure:PC governance, PC workforce development, and economic conditions of PC. The strength of PC services delivery process was measured by 4 dimensions of: accessibility, continuity, coordination, andcomprehensiveness of PC. The PC dimensions were operationalizedby a total of 77 indicators for which data was collected in 31European countries. The results show variation in PC strengthacross Europe, indicating a discrepancy in the responsibility given toPC in (inter)national policy initiatives and the needed investments inPC to solve e.g. future shortages of workforce. Countries are consistent in their PC focus on all important structure dimensions.Countries need to improve their PC information infrastructure tofacilitate PC performance management. This study was able toshow that strong PC has a positive impact on population health,reducing disparity in health, and avoiding unnecessary hospitalizations.
