Orbital complications of acute sinusitis: Evaluation, management, and results

Öz:
Amaç: Akut rinosinüzit orbital komplikasyonları çok sıkgörülmemekle birlikte, enfeksiyonun orbita ve intrakra-niyal dokulara hızla yayılması ile ciddi ve mortalitesiyüksek komplikasyonlar olarak ortaya çıkabilir. Bu çalış-manın amacı, akut rinosinüzit komplikasyonu nedeniylekliniğimizde yatarak tedavi edilen olguların tıbbi ve cer-rahi sonuçlarını gözden geçirmektir.Yöntemler: Kliniğimize akut rinosinüzit komplikasyonunedeniyle Ocak 2010 ve Mart 2012 tarihleri arasındabaşvuran olgular incelendi. Çalışmada, akut rinosinüzitkaynaklı orbital komplikasyonlar nedeniyle tedavi edi-len 11 olgu; 8 (%73) erkek, 3 (%23) kadın ortalama yaş12.36 (yaş dağılımı 6-20) retrospektif olarak değerlen-dirildi. Olgular yaş, cinsiyet, etyolojik etkenler, yerleşimyerleri, tedavi yöntemleri ve sonuçlarıyla değerlendirildi.Bulgular: 9u (%82) preseptal sellülit, 1i (%9) orbitalapse ve 1i de (%9) subperiostal apse tanısıyla raporlandı. 11 hastanın 10unda mevcut şikayetler medikal tedaviyletam düzelme sağlanırken, bir hastada subperiostal apsenedeniyle sağ fonksiyonel endoskopik sinüs cerrahisi vesağ subperiostal apse drenajı uygulandı. Hastaların orta-lama yatış süresi 7 gün olarak değerlendirildi.Sonuç: Orbita; paranazal sinüslere ve özellikle etmoidhücrelere yakın komşuluğu nedeniyle rinosinüzit komp-likasyonlarının en sık görüldüğü bölgedir. Erken tanı veagresif tedavi istenilmeyen sonuçların önlenilmesindeanahtar rol oynamaktadır. Orbital enfamasyon varlığında kontrastlı Bilgisayarlı Tomografi görüntülemesi halaen uygun yöntemdir. Subperiostal orbital abse ve orbitaabse, görüntüleme teknikleri ile tespit edildiğinde cerrahi olarak tedavi edilmelidir.

Objective: Even though acute rhinosinusitis orbitalcomplications are not very common, they can appear asvery severe complications with high mortality rates, dueto infection spreading to orbital and intracranial tissues.Te objective of this study is to assess the treatment andits results for patients treated in our clinic due to rhi-nosinusitis complications.Methods: Patients who were admitted to our clinic dueto acute rhinosinusitis complications between January2010 and March 2012 were examined. We retrospec-tively evaluated 11 patients 8 (73%) males and 3 (23%)females with a mean age 12.36 years (range 6-20 years)who were treated for orbital complications of acute rhi-nosinusitis. Cases were evaluated according to age, sex,etiologic factors, localization, treatment, and outcome.Results: Preseptal cellulitis was observed in 9 patients(82%), orbital abscess was observed in 1 patient (9%), and subperiosteal abscess was observed in 1 patient(9%). Ten of 11 patients achieved full recovery throughmedical treatment, whereas 1 patient was treated withright subperiosteal abscess drainage and right functionalendoscopic sinus surgery due to subperiosteal abscess.Te patients average admission duration was 7 days.Conclusion: Te orbita is the area with the most com-mon sinusitis complications, due to its proximity to theparanasal sinuses and especially to ethmoid cells. Earlydiagnosis and aggressive treatment are important forthe reduction of unwanted manifestations. Computedtomography with contrast remains the optimal imagingstudy for orbital infammation. Surgical treatment is in-dicated when subperiosteal orbital abscess and orbitalabscess are scenes.
