Supragingival dental plak ve gastrik mukozada Helicobacter pylori'nin tespiti

Öz:

Gram-negatif bir çomak olan Helicobacter pylori'nin (H. pylori) aktif kronik gastrite neden olduğu ve insan tip B gastriti ve duodenal ülserin primer etiyolojik kaynağı olduğu bilinmektedir. Mikrobiyal dental plakta tespit edilene kadar insan ve hayvan midelerinin H. pylori için tek rezervuar olduğu düşünülmekteydi. Çalışmamızda bu bakterinin bulaşma yolunun incelenmesi amacıyla dispeptik semptomlar gösteren ve endoskopiden en az 3 ay öncesine kadar antibiyotik ya da bizmut tuzu kullanmamış olan 26 hastanın gastrik mukoza ve supragingival dental plak örnekleri incelenmiştir. Hastalar oral hijyen ve periodontal sağlık açısından da değerlendirilmişlerdir. Gastrik endoskopik örneklerin H. pylori tespiti histolojik ve Campylobacter-like Organism (CLO) Testi ile analiz edilmiştir. Mikrobiyal dental plak örneklerindeki H. pylori varlığı da sadece CLO Testi ile saptanmıştır. Gastrik biyopsi örneklerinin %76.9'u H. pylori pozitif olan hastaların %60'ınm mikrobiyal dental plakları H. pylori pozitif olarak bulunmuştur. H. pylori negatif gastrik biyopsi örneklerinin %33.3'ünde supragingival dental plak H. pylori için pozitif olarak tespit edilmiştir. Hem gastrik hem de supragingival dental plak örnekleri H. pylori pozitif olan bireylerin fazlalığı supragingival dental plağın rezervuar görevi yaptığı ve gastrik mukoza için potansiyel reenfeksiyon kaynağı oluşturduğu hipotezini desteklemektedir.

Detection of Helicobacter pylori in supragingival dental plaque and gastric mucosa

Öz:

Helicobacter pylori (H. pylori), which is a Gram-negative rod has been shown to cause active chronic gastritis and has been implicated as a primary etiologic factor in human type B gastritis and duodenal ulcer disease. Human and animal stomachs were considered the only reservoirs of H. pylori until it was identified in human dental plaque. To determine the possible way of transmission of this bacterium, gastric biopsies and supragingival dental plaque were analysed from 26 patients with dyspeptic symptoms who'didn't use any antibiotic or bismuth salt for at least 3 months prior to endoscopy. Patients have been evaluated for oral hygiene and periodontal disease status. Gastric endoscopic specimens were analysed histologically and with Campylobacter-like Organism (CLO) Test for the detection of H. pylori. The presence of H. pylori in supragingival dental plaque was also assessed with CLO test. 76.9% of the gastric specimens were H. pylori positive, 60% plaque samples of these patients were H.p positive. For the H. pylori negative gastric specimens the percentage of H. pylori positive plaque sample was 33.3. The rate of both gastric and supragingival H. pylori positive results improved the hypothesis that supragingival dental plaque acts as a permanent reservoir and a potential source of reinfection for the gastric mucosa.
