Spontaneus Intracranial Hypotension: Diagnosis and Treatment of 2 Cases

Öz:

İntrakraniyal hipotansiyon, beyin omurilik sıvısı (BOS) volümü ya da basıncının azalmasınabağlı ortaya çıkan, ortostatik baş ağrısı ile prezente olan klinik bir sendromdur. Tanıda tipikklinik prezentasyon ve BOS basıncındaki azalmanın gösterilmesi ile birlikte radyolojikbulguların ortaya konulması anahtar rol oynar. Tedavi seçenekleri arasında yatak istirahatı,sıvı replasmanı, kafein, teofilin, abdominal bandaj yer alır, bu yöntemlerle başarılıolunamazsa uygulanma sıklığı ve önemi gün geçtikçe artan, altın standart kabul edilenepidural kan yaması tedavi alternatifidir. Biz bu makalede tedaviye dirençli olup epidural kanyaması ile tedavi ettiğimiz farklı etyoloji ve radyolojik görüntüleme özelliklerine sahip ikiolgunun tanı ve tedavisinin yaklaşımını tartışacağız.

Öz:

Intracranial hypotension is a clinical syndrome of decrease in volume or pressure ofcerebrospinal fluid (CSF), presenting with orthostatic headache. It is diagnosed by typicalclinical presentation and documentation of decrease in CSF pressure, along with theradiological findings. The first treatment option is conservative; bed rest, fluid replacement,caffeine, theophylline, abdominal bandage. If pain persists, epidural blood patch is consideredthe gold standard treatment. In this article patients resistant to medical treatment were treatedwith epidural blood patch for different etiologies; also diagnosis, radiological features andtreatment approaches were discussed.
