Comparison of Microleakage of Self-Adhesive Flowable Composites in Class V Cavities

Öz:
Amaç: Bu in vitro çalışmanın amacı; sınıf V kavitelere uygulanan kendinden bağlanabilen akışkan kompozitler ile üç farklı adeziv sistemle (üç basamaklı asitlenen ve yıkanan adeziv, iki basamaklı kendinden asitli adeziv ve bir basamaklı kendinden asitli adeziv) birlikte uygulanan akışkan kompozitlerin mikrosızıntı değerlerinin karşılaştırılmasıdır.Gereç ve Yöntem: Çürüksüz insan molar dişlerinin bukkal ve lingual/palatinal yüzeylerinde yetmiş beş adet standart sınıf V kavite hazırlandı. Kavitelerin oklüzal kenarları minede, gingival kenarları dişeti kenarından mine-sement birleşiminin 1 mm altında olacak şekilde yer aldı. Preperasyonu tamamlanan örnekler, rastgele 5 alt gruba ayrıldı: (1) Kendinden bağlanabilen akışkan kompozit (Fusio Liquid Dentin, FLD), (2) Kendinden bağlanabilen akışkan kompozit (Vertise Flow, VF), (3) Üç basamaklı asitlenen ve yıkanan adeziv/Akışkan kompozit (Optibond FL/Clearfil Majesty Flowable, 3ER-AK), (4) İki basamaklı kendinden asitli adeziv/Akışkan kompozit (Clearfil SE Bond/Clearfil Majesty Flowable, 2SE-AK), (5) Bir basamaklı kendinden asitli adeziv/Akışkan kompozit (Clearfil S3 Bond Plus/Clearfil Majesty Flowable, 1SE-AK). Termal siklüs ve %0.5'lik bazik fuksin boya uygulamasından sonra, örneklerden kesitler alındı ve stereomikroskop altında boya penetrasyon dereceleri skorlandı (n=30). Sonuçlar istatiksel olarak, Kruskal-Wallis, Mann-Whitney U ve Wilcoxon Signed Ranks testleri uygulanarak değerlendirildi. Bulgular: Kavitelerin mine yüzeyinde; VF, FLD, 3ER-AK grupları arasında istatistiksel olarak anlamlı bir fark gözlenmezken, 2SE-AK ve 1SE-AK grupları istatistiksel olarak anlamlı derecede daha yüksek mikrosızıntı değerleri gösterdi (p < 0.05). Kavitelerin dentin yüzeyinde ise 3ERAK ve 2SE-AK grupları, VF, FLD ve 1SE-AK gruplarından istatistiksel olarak anlamlı derecede düşük mikrosızıntı değerleri gösterdi (p < 0.05). 2SE-AK grubu mine yüzeyinde daha yüksek mikrosızıntı değeri gösterirken (p < 0.05), VF,FLD, 1SE-AK grupları dentin yüzeyinde daha fazla mikrosızıntı değeri gösterdi (p < 0.05).Sonuçlar: Kendinden bağlanabilen akışkan kompozitlerin her ikisi de mine yüzeyinde üç basamaklı asitlenen ve yıkanan adeziv/akışkan kompozitle benzer mikrosızıntı değeri göstermiştir. Dentin yüzeyinde ise kendinden bağlanabilen akışkan kompozitler, üç basamaklı asitlenen ve yıkanan adeziv/akışkan kompozit ve iki basamaklı kendinden asitli adeziv /akışkan kompozit gruplarına göre daha fazla mikrosızıntı oluşturmuştur.

Objective: The aim of this in-vitro study was to compare the microleakage of self-adhesive flowable composites to their counterparts applied with three different adhesive systems (3-step etch & rinse adhesive, 2-step self-etch adhesive, and 1-step self-etch adhesive) in Class V cavities.Materials and Methods: Seventy-five standardized Class V cavities were prepared on both buccal and lingual/palatinal surfaces of extracted non-carious human molar teeth. The occlusal margins were located on the enamel where the gingival margins were extended 1 mm below the cemento-enamel junction. Following preparation, all specimens were randomly divided into five groups: (1) self-adhesive flowable composite (Fusio Liquid Dentin, FLD), (2) selfadhesive flowable composite (Vertise Flow, VF), (3) 3-step etch & rinse adhesive/Flowable c omposite (Optibond FL/Clearfil Majesty Flowable, 3ER-AK), (4) 2-step self-etch adhesive/Flowable composite (Clearfil SE Bond/Clearfil Majesty Flowable, 2SE-AK), (5) 1-step self-etch adhesive/Flowable composite (Clearfil S3 Bond Plus/Clearfil Majesty Flowable, 1SE-AK). After thermocycling and immersion in a 0.5% basic fuchsin, the restorations were sectioned and dye penetration was evaluated under a stereomicroscope (n=30). The data were statistically analyzed with Kruskal-Wallis, Mann-Whitney U and Wilcoxon Signed Ranks tests (?=0.05).Results: At the enamel interface, no significant differences were observed between the microleakage scores of VF, FLD and 3ER-AK, while they showed significantly lower leakage scores than 2SE-AK and 1SE-AK (p < 0.05). At the dentin interface, 3ER-AK and 2SE-AK exhibited lower leakage scores than VF, FLD and 1SE-AK (p < 0.05). VF, FLD, 1SE-AK had higher leakage scores at the gingival margins (p < 0.05), while higher scores were detected at the enamel margins in 2SE-AK (p<0.05).Conclusions: Both self-adhesive flowable composites showed similar microleakage scores to the 3-step etch & rinse adhesive/flowable composite in enamel, whereas their performance in dentin was worse than 3-step etch & rinse adhesive/flowable composite and 2-step self-etch adhesive/flowable composite.
