Öz:

Kemoterapi ilişkili periferal nöropati (KİPN) kanser tanısı ve tedavisine bağlı gelişebilen önemli toksisitelerden birisidir. Tedavide nörotoksik ajanın kullanılması, kümülatif ilaç dozu, kombine kemoterapi, kemoterapötik ajanların hızlı uygulanması, eş zamanlı kemoterapi ve kraniyal radyoterapi, intrakarotid ve intratekal kemoterapi uygulamaları KİPN gelişme riskini önemli düzeyde artırmaktadır. Kemoterapi ilişkili periferal nöropati gelişimi kemoterapi protokolünün değişmesine, tedavi dozunun sınırlanmasına ya da tedavinin erken dönemde sonlandırılmasına neden olmaktadır. Nörotoksik olmayan ajanların seçilmesi, ilaç dozunun azaltılması ya da nörotoksik tedavinin kesilmesiyle KİPN önlenebilmektedir. KİPN şikayetlerinin yönetiminde farmakolojik ve farmakolojik olmayan yaklaşımların etkinliğine ilişkin çalışmalar oldukça sınırlı sayıdadır. Akut veya kümülatif nörotoksisite riski söz konusu olan hastalarda nörotoksisitenin erken dönemde saptanması, geri dönüşümsüz hasar ve travmaların önlenmesi, günlük yaşam aktivitelerinin sürdürülmesi ve öz bakım aktivitelerinin güvenli şekilde gerçekleştirilmesi için hasta-aile eğitimine önem verilmelidir. Bu derlemede, periferal nöropatinin önlenmesi, nöropatiyle ilişkili olumsuz etkilenmenin azaltılması ve yaşam kalitesinin geliştirilmesi sürecinde hemşirelerin rollerine ve hasta eğitimine dikkat çekilmesi amaçlanmaktadır.

Chemotherapy-Induced Peripheral Neurotoxicity and Nursing Interventions for Management of Peripheral Neurotoxicity

Öz:

Chemotherapy-induced peripheral neuropathy (CIPN) is one of the major toxicities that might develop due to cancer diagnosis and its treatment. The use of neurotoxic agents in the treatment, cumulative dose of the drug, combined chemotherapy, rapid implementation of chemotherapeutic agents, simultaneous chemotherapy-cranial radiotherapy, intracarotid and intrathecal chemotherapy significantly increase the risk of developing CIPN. Development of chemotherapy-induced peripheral neuropathy leads to a change in chemotherapy protocol, limiting of the therapeutic dose, or early termination of the therapy. CİPN can be prevented by uUsing nonneurotoxic agents, reducing the drug dose or discontinuation of neurotoxic therapy is effective for the prevention of CIPN. The number of studies on the effectiveness of pharmacological and nonpharmacological approaches to the management of neuropathy complaints is relatively limited. Patient-family education should be emphasized in patients carrying a risk of acute or cumulative neurotoxicity in order to detect neurotoxicity at the relatively early stages, to prevent irreversible damage and trauma, and to maintain activities of daily living, and to help patients safely execute the selfcare activities. This paper aims to address the interventions for controlling peripheral neuropathy, reducing the negative effects associated with neuropathy, and improving the quality of life, as well as to emphasize the patient education
