Changes in resistance percentage to antibiotics in Pseudomonas aeruginosa and Acinetobacter baumannii strains isolated from blood cultures of intensive care unit patients

Öz:
Amaç: Pseudomonas aeruginosa ve Acinetobacterbaumannii enfeksiyonları özellikle yoğun bakımünitesinde (YBÜ) yatan hastalar için en önemli sorunlardanbirisidir. Bu çalışmanın amacı, YBÜ hastalarında kandolaşımı enfeksiyonlarına sebep olan P. aeruginosa veA. baumanni etkenlerinin antimikrobiyal dirençpaternlerini belirlemek ve ampirik tedavi protokollerininuygunluğunu değerlendirmektir.Yöntemler: Ocak-Aralık 2010 ve Ocak-Aralık 2011tarihlerinde YBÜ'de yatan hastalara ait hemokültürörneklerinde üreyen suşların direnç oranları ayrı ayrıincelenerek karşılaştırıldı. Bu iki zaman aralığı arasındadirenç oranlarındaki farklılıklar karşılaştırılarakistatistiksel olarak analiz edildi.Bulgular: P. aeruginosa suşlarının piperasilintazobaktam,sefaperazon - sulbaktam, seftazidim,siprofloksasin, gentamisin, amikasin ve netilmisinedirenç oranlarında 2011 yılında 2010 yılına oranlaazalma olduğu saptandı (p değerleri sırasıyla, 0,0059,0,0000, 0,0048, 0,0350, 0,0000, 0,0000, 0,0003). Bunakarşılık, aztreonam direnç oranında artış saptandı(p değeri, 0,0155). İmipenem direncinin benzer oranlarda olduğu görüldü. A. baumannii suşlarının sefepim veamikasin direnç oranlarında ikinci periyotta ilkine oranlaistatistiksel olarak anlamlı azalma saptandı (p değerleri,0,0003 ve 0,0000). Ampisilin-sulbaktam, piperasilintazobaktamve imipeneme karşı direnç oranlarındaartış saptandı (p değerleri sırasıyla, 0,0003, 0,0210,0,0033). Her iki bakteri türünde de kolistine dirençsaptanmadı. A. baumannii izolatlarında tigesiklin direncisaptanmadı.Sonuç: Her hastanenin özellikle yoğun bakımbirimlerinden izole edilen suşların antibiyotik dirençpaternlerinin aktif sürveyansla takibi, ampirik tedaviyaklaşımlarını belirlemeye hizmet eder. Bu çalışmadaantibiyotik kullanım politikasının hastane enfeksiyonlarıile mücadelede önemli bir adım olduğu vurgulanmıştır.Sonuç olarak, direnç oranlarını azaltmak için, enfeksiyonkontrol önlemleri alınmalı, ampirik tedavi rejimlerisürekli gözden geçirilmeli ve aktif surveyans verilerinegöre belirlenmelidir.

Yoğun bakım hastalarının kan kültürlerinden izole edilen Pseudomonas aeruginosa ve Acinetobacter baumannii izolatlarının antibiyotik direnç yüzdelerindeki değişim

Öz:
Objective: Infections of Pseudomonas aeruginosaand Acinetobacter baumannii are one of the greatestconcerns for hospitalized patients, particularly those inintensive care units (ICUs). The aim of this study was todetermine the antimicrobial resistance percentages andto assess empirical treatment options for bloodstreaminfections due to P. aeruginosa and A. baumannii strainsin ICU patients.Methods: Resistance percentages of strains isolatedin January- December 2010 and January- December 2011were separately analyzed and compared. The differencesin resistance percentages between two intervals wasstatistically analyzed.Results: A statistically significant decrease was foundin the resistance percentage of piperacillin-tazobactam,cefoperazone-sulbactam, ceftazidime, ciprofloxacin,gentamicin, amikacin and netilmicin in the second periodcompared with the first (p values were 0.0059, 0.0000,0.0048, 0.00350, 0.0000, 0.0000, 0.0003, respectively)for P. aeruginosa strains. Whereas resistance percentageof aztreonam was increased (p value was 0.0155).Resistance percentage of imipenem was found similar. In A.baumannii strains, a statistically significant decreasewas found in resistance percentage of cefepime andamikacin in the second period compared with the first(p values were 0.0003, 0.0000). Resistance percentageof ampicillin-sulbactam, piperacillin-tazobactam andimipenem was increased (p values were 0.0003, 0.0210,0.0033). There was no colistin resistance determined inboth species. Tigecycline resistance was not found in A.baumannii isolates.Conclusion: Active surveillance of antibioticresistance percentages of isolated strains especially inICUs serves to determine empirical treatment regimensin every institution. The present study emphasized thatantibiotic usage policy is an important step to combathospital infections. Consequently, infection controlmeasures should be taken, empirical treatment regimensshould be constantly reviewed, and should be determinedaccording to active surveillance data in order to decreaseresistance percentages.
