Keratokonus Hastasına Güncel Yaklaşım

Öz:
Keratokonus korneanın sıklıkla bilateral, görme kaybı ile gidebilen, ilerleyici ektazik bir hastalığıdır. Keratokonusun etyolojisi multifaktöryel olup, gerçek etyoloji tam olarak bilinmemektedir. Erken evre Keratokonus tanısı için güncel en iyi yöntem Scheimpflug korneal topografi veya optik koherans tomografidir. Keratokonusun güncel tedavisinde basamaklı olarakcerrahi olmayan ve cerrahi birçok yöntem kullanılmaktadır. Erken evrede refraktif hatayı düzeltmek için gözlük ve kontaktlensler tercih edilmektedir. Görme keskinliğinin gözlük veya kontakt lensler ile düzeltilemediği, progresyonun görüldüğü ortave ileri Keratokonus olgularında ise kollajen çapraz bağ, kornea içi halka, göz içi refraktif lensler, lameller keratoplasti vepenetran keratoplasti gibi güncel cerrahi yöntemler kullanılmaktadır.

Current Approach to Keratokonus Patient

Öz:
Keratoconus is a common bilateral and progressive ectatic disease of the cornea with vision loss. The etiology of keratoconusis multifactorial and the true etiology is not fully known. Current best method for early stage keratoconus diagnosis isScheimpflug corneal topography or optical coherence tomography. In the current tratment of keratoconus, many non-surgical and surgical methods are used. In early stage, glasses and contact lenses are preferred to correct refractive errors. In thecases of moderate and advanced keratoconus in which visual acuity can not be corrected with glasses or contact lenses, progressive surgical techniques such as collagen cross-linking, refractive intraocular lenses, lamellar keratoplasty and penetrating keratoplasty are used.
