Legg-Calve-Perthes Hastalığında Staheli Shelf Asetabuloplasti Uygulaması

Öz:

Amaç: Legg-Calve-Perthes hastalığının başlangıç ve fragmantasyon dönemlerinde uyguladığımız Staheli Shelf Asetabuloplasti yönteminin lateral kolon tip B/C ve C hastalardaki kısa dönem sonuçlarını incelemeyi ve literatür ışığında tartışmayı amaçladık.Gereç ve Yöntem: Şubat 2004 ile Mart 2009 tarihleri arasında Staheli Shelf Asetabuloplasti ile tedavi edilen lateral kolon tip B/C ve tip C hastaları retrospektif olarak incelendi. Hastaların ameliyat öncesi klinik ve radyolojik verileri son kontrol muayenesindeki verileri ile karşılaştırıldı. Hastaların kalça eklem hareket açıklıkları, artikülotrokanterik mesafe (ATM), merkez kenar açısı (MKA), femur başı büyüklüğü oranı (FBB), asetabüler örtünme miktarı (AÖM) ve medial eklem aralığı mesafesi (MEAM) karşılaştırmalı olarak değerlendirildi. Lateral kolon tip B/C ve tip C hastaların cerrahi tedavi sonrası klinik ve radyolojik bulguları değişim miktarları karşılaştırıldı.Bulgular: Çalışmaya dahil edilen 16 erkek hastanın ameliyat sırasındaki ortalama yaşı 9.1±1.7 yıldı. Tüm hastalar başlangıç ve fragmantasyon evresindeydi. Lateral kolon sınıflamasına göre 6 hasta tip B/C, 9 hasta tip C olarak değerlendirildi. Hastaların son kontrollerindeki kalça hareket açıklıkları ameliyat öncesine göre istatistiksel olarak anlamlı derecede yüksekti (p<0.001). ATM (p=0.085) ile FBB (p=0.725) parametrelerindeki azalmanın istatistiksel olarak anlamlı değildi. MKA (p<0.001), AÖM (p<0.001) parametrelerindeki artış ile MEAM (p=0.01) parametrelerindeki azalma istatistiksel olarak anlamlı değildi. Lateral kolon B/C hastalardaki kalça fleksiyon derecesi değişim miktarı, tip C hastalardaki değişim miktarından istatistiksel olarak anlamlı derecede yüksekti (p=0.007). Ancak diğer parametrelerdeki değişim miktarları ise istatistiksel olarak anlamlı değildi (p>0.05).Sonuç: Staheli Shelf Asetabuloplasti ameliyatının komplikasyonunun az, klinik ve radyolojik sonuçlarının başarılı olması nedeni ile Perthes hastalığı cerrahi tedavisinde akılda tutulmasını önermekteyiz. Ancak, henüz uzun dönem takipleri ile ilgili sonuçlarının olmaması nedeni ile kontrol grubu olan ileri çalışmaların yapılması gerektiğini düşünmekteyiz.

Staheli Shelf Acetabuloplasty Procedure in Legg-Calve-Perthes Disease

Öz:

Objective: We aimed to evaluate short- term results of Staheli shelf acetabuloplasty procedure which we performed at initial and fragmentation stages of Legg-Calve-Perthes disease for lateral pillar type B/C and C patients and to discuss the results in the light of literature.Material and Methods: Between February 2004 and March 2009, lateral pillar type B/C and type C patients treated by Staheli shelf acetabuloplasty were evaluated retrospectively. Preoperative clinical and radiological data of patients were compared with the data at final follow- up examination. Hip range of motions, articulotrochanteric distance (ATD), center edge angle (CEA), femoral head size ratio (FHS), acetabular coverage amount (ACA) and medial joint space distance (MJSD) of patients were compared. Changes in measure of clinical and radiological findings of lateral pillar type B/C and C patients were also compared postoperatively. Results: The mean age of 16 male patients who were included in the study was 9,1 years at the time of surgery. All of the patients were in initial and fragmentation stages. According to lateral pillar classification, 6 patients were evaluated as type B/C and 9 patients were evaluated as type C. The hip range of motions of the patients at final follow up were statistically higher than preoperative measurements (p<0.001). Decrease of ATD (p=0.085) and FHSR (p=0.725) parameters were not statistically significant. Increases in CEA (p<0.001), ACA (p<0.001) parameters and decrease of MJSD (p=0.01) parameters were not statistically significant. Changes of hip flexion degrees in lateral pillar B/C patients were statistically higher than in type C patients (p=0.007). But changes of other parameters were not statistically significant (p>0.05).Conclusion: We recommend that Staheli shelf acetabuloplasty procedure should be kept in mind for the surgical treatment of Perthes disease in that these procedure has lower complication rates and successful clinical and radiological outcomes. Since its longer follow- up results are not available, further controlled researches should be done.
