Çocuklarda Ağrı Değerlendirme ve Ölçekleri: Kültür ve Yaşın Ağrı Değerlendirmesine Etkileri

Öz:
Çocukların hastaneye başvurmalarına neden olan en yaygın olgulardan biri, evrensel deneyimolan ağrı varlığıdır. Ağrı çok yaygın olmasına rağmen öznel ve kişisel bir deneyimdir. Ağrının en doğruşekilde anlaşılması ancak kişinin kendiraporu ile mümkündür. Çünkü insanlar yapısal olarak aynı ağrısistemine sahip olsa da ağrı sinyallerinin talamustan duyusal kortekse iletimi sırasında oluşan ağrı algılamasüreci, gelişimsel ve kültürel faktörler dâhil birçok etmene göre şekillenmektedir. Bu kişiselsürecin şekillenmesine etki eden faktörler, kişilerin ağrı ifadelerine ve davranışlarına da önemli derecedeetki etmektedir. Kliniklerde ise sağlık çalışanları sıklıkla çocuklarda ağrı problemi ile karşı karşıyakalmaktadırlar ve ağrı tedavisine başlamadan önce ağrıyı ölçmeleri gerekmektedir. Fakat, ağrı algıve ifadesi kültür ve yaşa göre farklılık göstermesi yanı sıra; çocuklar, gelişimsel özellikler nedeni ile ağrılarınıerişkinler gibi ifade edebilecek iletişim kâbiliyetine sahip değillerdir. Tüm bunlar, çocuklardaağrı değerlendirme sürecini çok daha karmaşık ve zor bir hâle getirmektedir. Bu nedenlerden dolayı,pediatrik ağrı değerlendirme süreci çok daha hassas ve özel olmalıdır. Sonuç olarak; sağlık çalışanları,pediatrik ağrıyı kültür ve yaş gibi faktörleri göz önüne alarak nasıl değerlendirebileceği ile ilgili bilgiyesahibi olmalıdırlar. Çocuklara özel ağrı değerlendirme yöntemleri ve ölçekleri yaşa ve kültürel faktörlerebağlı olarak, geçerlilik ve güvenirlilik ölçütleri belirlenerek kullanılmalıdır.

Pediatric Pain Assessment and Tools: The Influence of Culture and Age on Pain Assessment

Öz:
Pain is one of the most common universal phenomenon that children visit hospitals. Although pain is very common, it is highly subjective and individual experience. The most reliable method to understand pain depends on individual’s report. Since, although each individual has same pain structures, the transmission of pain signals from thalamus to sensory cortex, named as pain perception, influences from several factors, including cultural and developmental variables. These individual factors, therefore, influence pain expression and behaviors. In clinics, health care workers commonly face with pediatric pain and they need to assess pain before treatment. On the other hand, beside from cultural and age related factors, due to developmental reasons, children have limited communication skills. Assessment of pain in children, therefore, is more complex and difficult than adults. As consequence, pain assessment in children should be more sensitive and individual. Based on indicated reasons, health care workers should be aware of how pediatric pain should be assessed and measured according to age and cultural factors. Further to this, pain assessment tools should be tested according to validity and reliability properties when they will be used in a different culture.
