Geleneksel ve İmplant Destekli Overdenture Protez Kullanan Tam Dişsiz Hastaların Çiğneme Performansının İn Vivo Olarak Karşılaştırılması

Geleneksel ve İmplant Destekli Overdenture Protez Kullanan Tam Dişsiz Hastaların Çiğneme Performansının İn Vivo Olarak Karşılaştırılması
Öz: Amaç: Farklı alt çene kret yüksekliğine sahip tam protez (TP) veya implant destekli overdenture protez (İDOP) kullanan hastaların çiğneme performansının incelenmesi; çiğneme performansı ile maksimum ısırma kuvveti ve oklüzal temas alanı arasındaki ilişkinin değerlendirilmesidir.Gereç ve Yöntemler: Çalışma, alt üst TP veya üst tam alt İDOP yaptırmış 45 hasta üzerinde gerçekleştirilmiştir. Hastalar alt çene kretinin simfiz bölgesindeki kemik yüksekliğine göre yüksekkret, düşük kret ve İDOP kullanan düşük krete sahip hastalar olmak üzere üç gruba ayrılmıştır.Çiğneme performansının değerlendirilmesinde elek analiz yöntemi; maksimum ısırma kuvveti veoklüzal temas alanlarının belirlenmesinde ise basınca duyarlı Dental Pre-scale tip R-50 H (Fuji FilmCo., Tokyo, Japonya) filmlerinden ve sistem için özel hazırlanmış DentalPre-scale Occluzer (Dental Occlusion Pressuregraph FPD-703, Fuji Film Co, Tokyo, Japonya) görüntü tarayıcısından oluşan sistem kullanılmıştır. Elde edilen verilerin istatistiksel olarak karşılaştırılması tek yönlü varyansanalizi testi (ANOVA) ile çalışma parametreleri arasındaki ilişki Sperman’ın nonparametrik korelasyon testi ile yapılmıştır. Gruplar arası farklılıkların değerlendirilmesinde Duncan testi kullanılmıştır. Bulgular: Çiğneme performansı ve oklüzal temas alanı yönünden gruplar arasında istatistikselolarak anlamlı bir fark bulunmamıştır. İDOP kullanan hastalar en yüksek maksimum ısırma kuvvetine sahip grup olarak belirlenmiştir. Maksimum ısırma kuvveti, oklüzal temas alanı ve çiğnemeperformansı arasında istatistiksel açıdan bir ilişki bulunmamıştır. Sonuç: Çalışmamızda, mandibular rezorbe krete sahip hastalarda İDOP uygulanması maksimum ısırma kuvvetini artırmakla birlikte, çiğneme performansında herhangi bir değişiklik meydana getirmemiştir. Tam protezhastalarında kret yüksekliğinin ve İDOP tedavisinin çiğneme performansı ve oklüzal temas alanıüzerinde etkisinin olmadığı bulunmuştur.

An In Vivo Comparison of Masticatory Efficacy of Patients Using Conventional Dentures and Implant-Supported Overdentures
Öz: Objective: To examine the chewing performance of patients with different mandibular bone height using complete denture (CD) or implant retained overdenture (IROD); and to evaluate the relationship between chewing performance, maximum bite strength and occlusal contact area. Material and Methods: The study was carried out on 45 patients that received either a conventional or a mandibular IROD. Patients were divided into three groups according to mandibular bone height at semphisal region: high crest, low crest and low crest received an IROD. Sieve analysis method was used in the evaluation of chewing performance; pressure sensitive Dental Pre-scale Type R-50 H films and (Fuji film Co., Ltd. Tokyo, Japan) Dental Pre-scale occluzer scanner (Dental Occlusion FDP-703, Fuji Film Co., Tokyo, Japan) were used for determining maximum bite force and occlusal contact areas. The data obtained was statistical analysed by one-way variance analysis test (ANOVA) and Sperman's nonparametric correlation test. The Duncan test was used to evaluate the differences between groups. Results: There was no statistically significant difference between the groups in terms of chewing performance and occlusal contact area. Patients using IROD were identified as the group with the highest maximum bite force. There was no statistically significant correlation between maximum bite force, occlusal contact area and chewing performance. Conclusion: In the present study, maximum bite force was higher for patients received an IROD, although no change in chewing performance has occurred. It was found that crest height and IROD treatment had no effect on chewing performance and occlusal contact area in edentulous patients.
