An unusual thrombosis of an ostial left internal mammary artery graft causing acute coronary syndrome five years after coronary bypass surgery
Öz: For long-term patency, a left internal mammaryartery (LIMA) graft is considered the gold standard for leftanterior descending artery (LAD) revascularization. Subsequentocclusion of a LIMA graft may be related to atherosclerosis,narrowing at anastomotic locations, fixed torsionswithin the graft, straight tubular stenosis, dissection, or vasospasm.Late thrombotic occlusion of a LIMA, however, israre, and the cause is not known, though case reports canbe found in the literature. The present case is a descriptionof the successful revascularization via percutaneous interventionof an ostial LIMA thrombotic occlusion occurring 5years after bypass surgery in a 71-year old patient diagnosedwith acute coronary syndrome.

Öz: Sol ön inen arter revaskülarizasyonunda internal mammarian arter (İMA) grefti uzun süreli açıklık açısından altın standarttır. İnternal mammarian arter greft tıkanıklığı, ateroskleroz, anastamoz bölgesinde daralma, greft içinde açılı yerde daralma, düz tübüler daralma, diseksiyon ve spazm nedeniyle olabilmektedir. Geç İMA trombotik tıkanıklığı nadirdir, nedeni bilinmemekte olup, literatürde olgu sunumları bildirilmiştir. Biz mevcut olguda, baypas operasyonundan beş yıl sonra gelişen, 71 yaşındaki hastada akut koroner sendroma neden olan İMA proksimal osteal tromboze lezyonun başarılı perkütan revaskülarizasyonunu sunduk.
