Isotretinoin-associated possible Kounis syndrome: A case report and a review of other cardiovascular side effects reported in the literature

Isotretinoin-associated possible Kounis syndrome: A case report and a review of other cardiovascular side effects reported in the literature
Öz: Isotretinoin is widely used in the treatment ofacne vulgaris and other dermatological diseases. Numerousside effects have been reported in the literature. Amyocardial bridge occurs when segments of the coronaryartery create an intramyocardial tunnel. Atheroscleroticplaque formation frequently occurs in the segment proximalto a myocardial bridge. Coronary thrombus formation, whichis often the cause of myocardial infarction in young patients,can be triggered by many factors. Kounis syndrome is describedas acute coronary syndromes associated with allergicor hypersensitivity reactions. This article is a descriptionof the case of a patient predisposed to the development ofa thrombus by a myocardial bridge who was successfullytreated for coronary thrombosis and which may representa case of Kounis syndrome associated with isotretinoin usepresented in the context of the relevant literature.

Öz: İsotretinoin, akne ve diğer dermatolojik hastalıklarda yaygın olarak kullanılmaktadır ve literatürde birçok yan etki bildirilmektedir. Miyokardiyal köprüleşme, koroner arterin belli segmentinin intramiyokardiyal tünel biçimli seyir izlemesidir. Miyokardiyal köprünün oluştuğu koroner arter segmentinin proksimalinde sıklıkla aterosklerotik plak oluşumu izlenir. Genç hastalarda miyokart enfaktüsüne sıklıkla neden olan koroner arterde trombüs oluşumunu tetikleyen birçok faktör olabilmektedir. Kounis sendromu, alerjik ya da hipersensivite reaksiyonunun eşlik ettiği akut koroner olay olarak tanımlanmaktadır. Bu yazıda miyokardiyal köprüleşmenin zemin hazırladığı koroner trombüsün başarılı şekilde tedavi edildiği bir olgu isotretinoin ile ilişkili olası Kounis sendromu olarak tanımlanmış ve konu ile ilgili literatür eşliğinde sunulmuştur.
