Meme Kanseri Tedavisiyle İlişkili Lenfödemi Olan Hastalarda Lenfödem Şiddetinin Yaşam Kalitesi, Üst Ekstremite Fonksiyonu ve Fiziksel Aktivite Düzeyi Üzerindeki Etkileri

Öz:
Amaç: Bu çalışmada meme kanseri tedavisi ile ilişkili hafif, orta ve şiddetli lenfödemi olan hastalar arasındaki yaşam kalitesi, üst ekstremitenin fonksiyonel durumu ve fiziksel aktivite düzeyifarklılıklarını karşılaştırmak amaçlanmıştır.Gereç ve Yöntemler: Çalışmaya meme kanseri tedavisi ile ilişkili lenfödem tanısı olan toplam 83kadın hasta (%24 hafif, %41 orta, %35 şiddetli) alındı. Lenfödemin varlığı ve şiddeti çevre ölçümüile belirlendi. Lenfödem Yaşam Kalite Ölçeği (LYKÖ); Kol, Omuz ve El Sorunları Anketi’nin (KOESA) kısa versiyonu; ve Uluslararası Fiziksel Aktivite Anketi’nin kısa formu (UFAA-KF), sırasıylayaşam kalitesini, üst ekstremitenin fonksiyonel durumunu ve fiziksel aktivite düzeyini değerlendirmek için kullanıldı.Bulgular: Hafif, orta ve şiddetli lenfödemi olan hastalar arasında yaşam kalitesi ve üst ekstremitenin fonksiyonel durumu açısından istatistiksel olarak anlamlı fark vardı (p<0,05). Ancak fiziksel aktivite düzeyi bakımından üç grup arasında anlamlı fark yoktu (p>0,05). İkili analizlerinsonuçlarına göre, şiddetli lenfödemi olan hastalarda, yaşam kalitesinin fonksiyon, görünüm vesemptom alt alanlarının ve üst ekstremitenin fonksiyonel durumunun hafif derecede lenfödemiolan hastalara göre daha fazla etkilendiği bulundu (p<0,016). Lenfödem şiddeti ile LYKÖ fonksiyon (r=0,34), görünüm (r=0,32) ve semptom (r=0,30) alt alanları ve KOESA skorları (r=0,33)arasında istatistiksel olarak anlamlı ancak zayıf bir ilişki bulundu (p≤0,01).Tartışma ve Sonuç: Şiddetli lenfödemi olan hastalarda yaşam kalitesinin ve üst ekstremiteninfonksiyonel durumunun hafif lenfödemi olan hastalara göre daha fazla etkilendiği bulundu. Busonuçlar, meme kanseri cerrahisi geçiren bireylerde lenfödem şiddeti hafif düzeyden ileri düzeye geçtiğinde yaşam kalitesinin ve üst ekstremite fonksiyonelliğinin etkilenebileceğini göstermektedir. Dolayısıyla bu etkiyi önlemek veya azaltmak için lenfödemin erken teşhis ve tedavisi önemlidir.

Effects of Lymphedema Severity on Quality of Life, Upper Limb Function, and Physical Activity Level in Patients with Breast Cancer Treatment-related Lymphedema

Öz:
Aim: This study aimed to compare the differences in quality of life, upper extremity functional status, and physical activity level between patients with mild, moderate, and severe breast cancer treatment-related lymphedema. Materials and Methods: A total of 83 female patients with a diagnosis of breast cancer treatment-related lymphedema (24% mild, 41% moderate, and 35% severe) were included. Lymphedema presence and severity were evaluated by circumferential measurement. The Turkish versions of the Lymphedema Quality of Life (LYMQOL) and Quick Disability of Arm, Shoulder and Hand (DASH) questionnaires and the International Physical Activity Questionnaire-Short Form (IPAQ-SF) were used to assess quality of life, upper extremity functional status, and physical activity level, respectively. Results: There were significant differences in quality of life and upper extremity functional status between patients with mild, moderate, and severe lymphedema (p<0.05). However, there was no significant difference in physical activity level between the three groups (p>0.05). Based on the pairwise analysis findings, function, appearance, and symptom subscales of life quality and upper extremity functional status were found to be more affected in patients with severe lymphedema, compared to those with mild lymphedema (p<0.016). We observed significant but weak correlations between lymphedema severity and the LYMQOL function (r=0.34), appearance (r=0.32), and symptom (r=0.30) subscales, and the DASH (r=0.33) scores (p≤0.01). Discussion and Conclusion: It was found that life quality and upper extremity functional status were more affected in patients with severe lymphedema, compared to those with mild lymphedema. These results indicate that life quality and upper extremity functionality may be affected as lymphedema progresses from mild to severe stages in patients undergoing breast cancer surgery. Therefore, prevention and reduction of this effect depend on the early diagnosis and treatment of lymphedema.
