COVID-19 pandemisi ve çocuk cerrahisi

Öz:
2019 yılının Aralık ayında ilk olarak sebebi bilinmeyen pnömoni olarak rapor edilmesinden buyana, SARS CoV-2 veya daha yaygın bilinen adıyla COVID-19 hakkında bildiklerimiz gün geçtikçeartmakta ve aynı oranda da değişmektedir. COVID-19 salgını tüm dünyayı ve elbette dünyadakisağlık sistemlerini ve bu sistem içerisinde yer alan kurum ve kuruluşları olumsuz etkilemiştir.Bugüne kadar bu salgından etkilenenlerin tanısı, tedavisi ve tedavi süreci içerisinde yer alan cerrahlarile ameliyathane personelinin cerrahi öncesi, sırası veya sonrasında nasıl davranmalarıgerektiğine dair pek çok rapor, makale ve rehber yayınlanmıştır. Bu yazımızda COVID-19 pandemisidöneminde çocuk cerrahisinde, hasta ve sağlık personeli güvenliği yanında acil cerrahisorunları olan çocuklara zamanında cerrahi bakım sağlarken uyulması gereken önlemler, tartışılmaktadır.Güncel literatürü inceleyerek uluslararası basılmış makale, öneriler ve algoritmalar ile Bilim kuruluçalışması esas alınarak hazırlanmış Sağlık Bakanlığı’nın rehberleri irdelendi. Bu verilerden yolaçıkarak Çocuk cerrahlarının COVID-19 pandemisi döneminde takip edebilecekleri preoperatif veperoperatif rehber ve öneriler bir araya getirildi.Çocukluk çağında Covid-19 enfeksiyonunun asemptomatik de olabileceği göz önüne alınarak;elektif ameliyatların ertelenmesi, acil-elektif ameliyat kararının doğru verilmesi, ameliyat öncesihazırlık, ameliyatlarda koruyucu malzemelerin uygun kullanımı, ameliyatlarda kullanılacak malzemeve yöntemlerde özenli seçim, hasta ve ekip güvenliği için ana öğelerdir.

Since December 2019, when it was first reported as an atypical pneumonia with unknown source, our general knowledge on SARS CoV-2 or commonly known as COVID-19 is accumulating and changing day by day. COVID-19 pandemic has adversely affected the globe and global healthcare systems, health institutions and associated co-workers. To date, several reports, papers and guidelines published to discuss how to diagnose and treat this condition and how to surgeons or operating room crew should act pre- per- and post-operatively during such a pandemic. In this paper, we discuss the preventive measures and precautions that should take during COVID-19 pandemic in pediatric surgery have been evaluated, not only for children who needs prompt and proper emergency surgery but also for the patients’ and health workers’ safety. We reviewed current literature in terms of international published papers, recommendations, algorithms and Ministry of Health guidelines based on Scientific Committee Study. According the current data, we compiled Pre-operative and Per-operative guide and recommendations for Pediatric Surgeons to be follow during COVID-19 pandemic. Keeping in mind that children with COVID-19 infection might be asymptomatic; postponing the elective procedures and right decision making between emergent/urgent and elective procedures, preoperative preparation, accurate use of protective equipment, careful selection of surgical materials and operative methods are the key elements for patients’ an surgical team’s safety.
