A Drastic Decrease in Emergency Admissions in a German Child and Adolescent Psychiatry Clinic: A COVID-19-Pandemic Phenomenon or an Actual Decrease in the Number of Emergency Patients

Investigation of Psychiatric Comorbidity in Adolescents with Obesity

Öz:
Objectives: In our study, it was aimed to investigate the frequency and distribution of psychiatric disorders in adolescents with overweight/ obesity, and to compare psychiatric distribution with adolescents with normal weight. Materials and Methods: One hundred six adolescents between the ages of 12 and 18 years, who applied to the Pediatric Endocrinology Department of Süleyman Demirel University Hospital and who were diagnosed as overweight or obesity by an endocrinology specialist, were included in the study. The control group included 50 subjects who volunteered to participate in the study and were similar in age and gender to the study group and who did not receive any psychiatric treatment with a Body Mass index percentile below 85. Psychiatric evaluation was done using the Kiddie-schedule for affective disorders and schizophrenia-present and lifetime version. Results: Seventy (66%) of overweight/obesity cases received at least one, and 41 (38,7%) received two or more psychiatric diagnoses. The most common psychiatric disorders in the overweight/obesity group were attention-deficit/hyperactivity disorder (ADHD) (n=42, 39.6%), specific phobia (n=25, 23.6%) and social anxiety disorder (n=22, 20.8%), respectively. The diagnosis of ADHD (χ2=16.340, p<0.001), social anxiety disorder (χ2=9.507, p=0.002) and generalized anxiety disorder (χ2=3.978, p=0.046) were found to be higher in the group diagnosed with obesity. Conclusion: In our study, it was found that psychiatric comorbidities, especially ADHD and social anxiety disorder, were high in overweight/ obese adolescents. Our results suggest that adolescents diagnosed with overweight/obesity should necessarily undergo psychiatric evaluation.

Obezite Tanılı Ergenlerde Eşlik Eden Psikiyatrik Bozuklukların Araştırılması

Öz:
Amaç: Çalışmamızda aşırı kiloluluk/obezite tanısı olan ergenlerde psikiyatrik bozuklukların yaygınlığını ve dağılımını araştırmak, ayrıcanormal vücut ağırlığı olan ergenlerle psikiyatrik tanı dağılımlarını karşılaştırmak amaçlanmıştır.Gereç ve Yöntem: Çalışmaya Süleyman Demirel Üniversitesi Hastanesi Pediyatrik Endokrinoloji Bilim Dalı Polikliniği’ne başvuran ve birendokrinoloji uzmanı tarafından aşırı kiloluluk ya da obezite tanısı almış 12-18 yaşlar arası 106 ergen dahil edilmiştir. Kontrol grubuna iseçalışmaya katılmaya gönüllü olup, yaş ve cinsiyet bakımından araştırma grubu ile benzer olan, Vücut Kitle indeksi persentilleri 85’in altındaolan herhangi bir psikiyatrik tedavi almayan 50 ergen dahil edilmiştir. Okul çağı çocukları için Duygulanım bozuklukları ve şizofreni görüşmeçizelgesi-şimdi ve yaşam boyu şekli kullanılarak psikiyatrik değerlendirme yapılmıştır.Bulgular: Aşırı kiloluluk/obezite tanılı olguların 70’i (%66) en az bir, 41’i (%38,7) ise iki ve daha fazla psikiyatrik tanı almıştır. Aşırı kiloluluk/obezite tanılı grupta en sık tespit edilen psikiyatrik bozukluklar sırasıyla dikkat eksikliği hiperaktivite bozukluğu (DEHB) (n=42, %39,6), özgülfobi (n=25, %23,6) ve sosyal anksiyete bozukluğu (n=22, %20,8) olup, aşırı kiloluluk/obezite tanılı grupta DEHB (χ2=16,340, p<0,001), sosyalanksiyete bozukluğu (χ2=9,507, p=0,002) ve yaygın anksiyete bozukluğu (χ2=3,978, p=0,046) tanıları kontrol grubuna göre daha fazla tespitedilmiştir.Sonuç: Çalışmamızda aşırı kilolu/obez ergenlerde psikiyatrik eş tanıların, özellikle de DEHB ve sosyal anksiyete bozukluğunun yüksek olduğutespit edilmiştir. Sonuçlarımız aşırı kiloluluk/obezite tanısı olan ergenlerin mutlaka psikiyatrik değerlendirmesinin yapılması gerektiğinidüşündürmektedir.
