Felin İdiyopatik Sistitis Tedavisinde Güncel Yaklaşımlar
Öz: Evde yaşayan kedilere ait alt üriner sistem bulguları en az 80 yıldan beri veterinerliteratüründe yer almaktadır. Felin İdiyopatik Sistitis (FİS) etiyolojisi bilinmeyen ve yaygıngörülen bir hastalıktır. Birçok çalışmaya rağmen hastalığın patogenezi hakkında bilgilerimiz halakısıtlı olmakla beraber var olan çok ilgi çekici ve ilginç teorilerin doğrulanmaya ihtiyacı vardır.FİS’in patogenezinde idrar kesesi, nöroendokrin sistem ve kedinin yaşadığı çevre arasındakarmaşık bir ilişki bulunmaktadır. FİS teşhisi diğer Aşağı Üriner Sistem Belirtilerinin (AÜSB)dışlanması ile konulmaktadır. Hastalığın patogenezindeki bilgilerimizin azlığı ve etkili terapötikmetetlarıda hala prematüredir. En üst seviyedeki ve ispatlanmış çalışmalar ise FİS’li kediler içinçok amaçlı terapotik üriner mama, çevresel zenginleştirme ve yaş mama ile beslemenin hastalığınyönetimini desteklediğini ortaya koymuştur. Öte yandan kronik ve tedavi edilemeyen FİS’likedilerin Pandora sendromu yönünden ele alınmaları gerekliliği de akıldan çıkarılmamalıdır.

Öz: Signs referable to the lower urinary tract of indoor-housed cats have been described in the veterinary literature for at least 80 years. Feline idiopathic cystitis (FIC) is a common disease of unknown etiology. Despite numerous studies, our understanding of this disease remains poor and although there are some interesting and attractive theories about the pathogenesis of the disease, much remains to be confirmed. Complex interactions between the bladder, neuroendocrine system, and the cat’s environment seem to be involved in the pathogenesis of FIC. FIC is diagnosed by excluding other causes of lower urinary tract signs. Along with our poor understanding of the pathogenesis of disease, knowledge of effective therapeutic intervention is also premature. For cats with FIC, the highest grade of evidence supports nutritional management with a multipurpose prescription urinary diet, environmental enrichment, and feeding moist food. On the other hand, it should be kept in mind that cats with chronic and untreated FIS should be considered in terms of Pandora syndrome.
