İn vitro fertilizasyon tedavi sonucunun bağlanma, yılmazlık, stres düzeyi, depresyon, anksiyete ve somatizasyon ile ilişkisi

Öz:

Amaç: Çalışmamızın amacı İn Vitro Fertilizasyon (IVF) tedavisi gören kadınlarda depresyon, anksiyete, somatizasyon belirtileri, bağlanma özellikleri ve yılmazlık düzey-leri ile infertiliteye ilişkin global stres düzeyi arasında ilişki olup olmadığını araştırmaktır. Çalışmamızın diğer amacı ise IVF tedavi sonucu ile bağlanma özellikleri, yılmazlık, infertiliteye ilişkin global stres düzeyi, depresyon, anksiyete ve somatizasyon belirtileri arasında ilişki olup olmadığını değerlendirmektir. Yöntem: Kocaeli Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Ana Blim Dalı Üremeye Yardımcı Teknikler Merkezi’ne ardışık olarak başvuran ve IVF tedavisi için uygun görülen 88 kadın çalışmaya alınmıştır. Katılımcılara infertilite tedavi protokolü öncesinde sosyodemografik veri formu, Kısa Semptom Envanterinin (KSE) depresyon, anksiyete ve somatizasyon alt ölçekleri, Fertilite Sorun Envanteri (FSE), Yakın İlişkilerde Yaşantılar Envanteri (YİYE) ve Yetişkin Yılmazlık Ölçeği (YYÖ) uygulanmıştır. Katılımcılara IVF tedavi protokolü uygulandıktan 3 ay sonraki izlemde gebelik oluşup oluşmadığı değerlendirilmiştir. Bulgular: FSE toplam puanı ile YİYE kaygı (p<0,001, r=0,314) ve YİYE kaçınma (p<0,001, r=0,364) puanı arasında pozitif yönde anlamlı bir ilişki bulunmuştur. FSE toplam puanı ile YYÖ toplam puanı arasında ise negatif yönde bir ilişki (p<0,001, r=-0,434) olduğu belirlenmiştir. KSE anksiyete, depresyon ve somatizasyon alt boyut puanı, YİYE kaygı ve kaçınma puanı, YYÖ toplam ve alt boyut puanı ve FSE toplam puan ortalamalarının IVF tedavisi sonucuna göre farklılık göstermediği saptanmıştır. Sonuç: Çalışmamızın sonuçları yılmazlık düzeyi ile bağlanmada kaygı ve kaçınma boyutlarının infertiliteye ilişkin stres ile ilişkili olduğunu göstermiştir. Anksiyete, depresyon, somatizasyon, infertiliteye ilişkin global stres, bağlanma ve yılmazlık puanları IVF tedavi sonucu ile ilişkili bulunmamıştır. Yılmazlık düzeyini güçlendirecek psikosoyal müdahaleler ve bağlanma temelli terapötik yaklaşımlar infertiliteye ilişkin stresin azaltılmasına yardımcı olabilir.

Öz:

Objective: The aim of this study was to investigate the relationship of depression, anxiety, somatization symp-toms, attachment dimensions and resilience with infertilitiy related stress. Our second aim was to explore the associations of In Vitro Fertilisation (IVF) treatment out-come with attachment dimensions, resilience, infertility related stress, depression, anxiety, and somatization symptoms. Method: 88 women who were consecutively applied to the Kocaeli University Medical FacultyDepartment of Gynecology and Obstetric, Assisted Reproductive Techniques Center and found eligible for IVF treatment were inclu-ded in the study. Sociodemographic data form, depression, anxiety and somatization subscales of Brief Symptom Inventory (BSI), Experiences in Close Relationships Scale (ECR), TheResilience in Midlife Scale (RIM) and Fertility Problem Inventory (FPI) were administered to all participants before IVF treatment protocol. Participants were evalua-ted whether pregnancy occurred or not 3 months after the IVF treatment protocol was applied. Results: A sig-nificant positive correlation was found between the FSE total score and the ECR anxiety (p<0.001, r=0.314) and ECR avoidance (p<0.001, r=0.364) scores. A negative correlation was found also between the FSE total score and RIM total score (p<0.001, r=-0.434). It was found that BSI anxiety, depression and somatization scores, ECR anxiety and avoidance scores, RIM total and sub-scale scores and FSE total score averages did not differ according to IVF treatment outcome. Discussion: The results of our study showed that the anxiety and avoid-ance dimensions of attachment and resilience are related to infertility stress. Anxiety, depression, somatization, infertility related stress, attachment and resilience scores were not associated with IVF treatment outcome. Psychosocial interventions to strengthen resilience and attachment based therapeutic approaches may help to reduce infertility related stress.
