Travmatik Beyin Hasarında Derin Beyin Stimülasyonu

Öz:
Travmatik beyin hasarı (TBH) sonrası oluşan, mesleki, fiziksel ve bilişsel işlev kayıplarını azaltmak amacıyla çok farklı rehabilitasyonyöntemleri kullanılmaktadır. Uzun yıllardır yapılan araştırmalara rağmen TBH sonrası iyileşmeyi sağlayacak başarılı bir farmakolojiktedavi hâlâ tanımlanamamıştır. Son dönemlerde artan nörostimülasyon teknikleri ve beyin elektriksel aktivitesinin anlaşılmayabaşlanması ile birlikte TBH olgularında işlevsel düzelmeyi destekleyecek tedavi yaklaşımları gündeme gelmeye başlamıştır.Günümüzde hareket bozukluklarının cerrahi tedavisinde sıklıkla kullanılan derin beyin stimülasyonunda (DBS) amaç; stereotaktikyöntemle beyinde belli bölgelere yerleştirilen elektrotlar aracılığıyla beyin elektriksel aktivitesinde ortaya çıkan anormal uyarılarıbaskılayarak farmakolojik tedavilerin yetersiz kaldığı semptomları ortadan kaldırmaktır. Travmatik beyin hasarlı olgularda son yıllardaolgu bazlı çalışmalar olmakla beraber daha çok hayvan deneylerinde özellikle de serebrovasküler enfarkt modellerinde derin beyinstimülasyonunun etkisi çalışılmış ve olumlu sonuçlar bildirilmiştir.

Many different rehabilitation methods are used in order to reduce the occupational, physical and cognitive functional losses that occur after traumatic brain injury (TBI). Despite the research continuing for many years, a successful pharmacological treatment that will ensure recovery after TBI has still not been reported. Nowadays, with the increase in neurostimulation techniques and the understanding of brain electrical activity, treatment approaches to support functional improvement in TBI cases have started to appear in the literature. The aim of deep brain stimulation (DBS), which is currently used frequently in the surgical treatment of movement disorders, is to eliminate the symptoms that can only be treated inadequately through pharmacological methods by suppressing the abnormal stimuli arising in the electrical activity of the brain via electrodes placed in certain areas of the brain by stereotactic methods. Although there are some case-based studies in traumatic brain-damaged patients in recent years, the effect of deep brain stimulation has generally been studied in animal experiments, especially cerebrovascular infarction models, and promising results have been reported.
