Prematüre retinopatisinde risk faktörleri ve tarama sonuçları

Öz:

Amaç: Prematüre retinopatisi (PR) gelişimi için muhtemel risk faktörlerini değerlendirmek ve prematüre bebeklerin düzenli taramasını yapmak amaçlandı. Gereç ve Yöntem: Şubat 2001-Haziran 2004 arasında 36 gestasyonel hafta ve daha küçük olan prematüre bebekler prospektif olarak değerlendirildi. Üç yüz otuz beş bebek çalışma kapsamına alındı. PR gelişimi ile doğum ağırlığı, gestasyonal yaş ve diğer risk faktörleri arasındaki ilişki araştırıldı. Bulgular: Olguların doğum ağırlıkları 500-2400 gr (ortalama, 480.98±294.41gr), gestasyonel yaşları ise 26 -36 hafta (ortalama 32.08±2.08) arasındaydı. Çeşitli evrelerde PR, 335 bebeğin 81'inde (%24.18) saptandı. Doğum ağırlığı (ki kare: 34.98, sd:3, P<0.001) ve gestasyonel yaş (ki kare. 50.22, sd:2, P<0.001) ile PR gelişimi açısından ters bir orantı tespit edildi. Respiratuar distres sendromu, sürekli pozitif havayolu basıncı ile oksijen kullanımı (SPHB), intrauterin gelişme geriliği (IUGG), anemi, kan transfüzyonu ve apne gibi risk faktörleri ile PR arasında istatistiksel olarak anlamlı bir ilişki bulundu (ki kare: 22.51, sd: 6, P<0.001). Sonuç: PR'nin multifaktöryel bir hastalık olduğu bilinmektedir. PR'e bağlı körlüğün önlenmesinde en iyi strateji önlemdir. Bu da gestasyonel haftası 32 hafta üzerinde bile olsa bütün prematüre bebeklerin düzenli taranmaları ile mümkün olabilir.
