Spontan Koroner Arter Diseksiyonunda Güncel Yaklaşım

Öz:
Spontan koroner arter diseksiyonu özellikle genç kadın ve ciddi aterosklerotik risk faktörü taşımayan bireylerde akut koroner sendro‐mun önemli nedenlerindendir. Literatürde eski vaka serilerinde farkındalığın ve teknik imkanların yetersizliğine bağlı olarak spontan ko‐roner arter diseksiyonu nedenli akut koroner sendromlar daha düşük oranlarda bilinse de anjiyografinin ve intrakoroner görüntülemetekniklerinin yaygınlaşması ile spontan koroner arter diseksiyonu prevalansı artmaktadır. Etkilenen grup, patogenez ve tedavi algorit‐ması aterosklerotik koroner arter hastalığından farklı olarak değerlendirilmekle birlikte hala predispozan faktörler, patogenez, optimaltedavi ve uzun dönem sonuçları ile ilgili netleştirilememiş kısımlar bulunmaktadır.

Öz:
Spontaneous coronary artery dissection is an important cause of acute coronary syndrome, especially in young women and individuals who do not have serious atherosclerotic risk factors. Although acute coronary syndromes caused by spontaneous coronary artery dis‐ section are known at a lower rate in the literature due to insufficient awareness and technical facilities, the prevalence of spontaneous coronary artery dissection increases with the spread of angiography and intracoronary imaging techniques. Although the affected group, pathogenesis and treatment algorithm are different from atherosclerotic coronary artery disease, there are still unclear parts of predisposing factors, pathogenesis, optimal treatment and long‐term outcomes.
