Bipolar Bozukluk Tedavisinde Bilişsel Davranışçı Terapi

Öz:
Bipolar Bozukluk (BB) biyo-psiko-sosyal yaklaşımla ele alınırken ”biyo” vurgusunun ön planda olduğu bir hastalıktır. Ancak hastalık belirtilerinin, birbirinden farklı atak dönemlerinin özelliklerinden dolayı ilaç tedavisiyle idaresi zor olan ve çoğu zaman sadece ilaç tedavisinin yetersiz kaldığı bir durumdur. Görülme sıklığı ve ilaç tedavilerinin yetersizliği düşünüldüğünde BB’nin tedavisinde psikososyal müdahalelerin önemi inkâr edilemez. Ayrıca tedavi uyumsuzluğu da psikososyal olarak ele alınması gereken bir başka sorundur. Psikososyal müdahaleler arasında, Bilişsel Davranışçı Terapi (BDT), deneysel ve esnek olması gibi birçok özelliğiyle eşsiz bir yere sahiptir ve akut mani dönemi dışındaki dönemlerde kanıta dayalı bir güçlendirici tedavi olarak önerilir. Bu gözden geçirmede BDT ile ilgili kanıtlara genel bir bakıştan sonra BDT yöntemlerinin hangi durumlarda nasıl kullanıldığıyla ilgili ayrıntılara yer verilmiştir. Bilişsel davranışçı perspektiften pragmatik bir yaklaşımla BB’de psikoterapi uygulamasının temel esaslarına odaklanılmıştır.

Cognitive Behavioral Therapy in Treatment of Bipolar Disorder

Öz:
Biological underpinnings (i.e.,”bio” of bio-psycho-social approach) of Bipolar Disorder (BD) has comparatively been more prominent when addressing to its etiology and treatment. Medical management of the disorder is usually difficult because of significant shortcomings of drug treatment and heterogeneity of symptoms and episodes with opposing features. Taking together the prevalence of BD and shortcomings of drug treatment, psychosocial interventions have undeniable importance in the treatment of the disorder. Another issue one must take into account is lack of treatment adherence. Cognitive Behavioral Therapy (CBT) has its unique position among psychosocial interventions with its features like being empirical and flexible. Accordingly, it is recommended as an adjuvant therapy in every stage of the disorder except acute mania. In this review, after generally addressing the evidence for CBT in BD, we discussed on how it is used in specific areas of the disorder. We focused on essentials of psychotherapy practice with a pragmatic approach from CBT point of view.
