Stomalı hastaların yaşadıkları cinsel sorunlar:Sistematik çalışma

Öz:

AMAÇ: Bağırsak stoması, birey için çeşitli günlük yaşam ile ilgili zorlukla rın yanında cinsel yaşamda sorunlara neden olan büyük bir etkiye sahiptir.Bu sistematik çalışmanın amacı, stomalı bireylerin cinsel yaşam sorunlarınıbelirlemektir. GEREÇ ve YÖNTEMLER: 2000–2020 yılları arasında yayınlanan makale leri içeren PubMed (MEDLINE), CINAHL, Cochrane Library, PsycINFO,Google schoolar veri tabanlarının aramalarına dayalı olarak sistematik birinceleme yapılmıştır. Çalışma için, kolostomi ve ileostomi cerrahisi geçirenyetişkin hastalar ile cinsel yaşama spesifik ölçekler kullanılarak yapılan oriji nal bir çalışmanın verilerini bildiren makaleler, dahil edilme kriterleri olarakbelirlenmiştir. BULGULAR: Belirlenen 1977 makaleden yedi tanımlayıcı, bir prospek tif, iki deneysel yönteme sahip toplam 10 makale çalışmaya dahil edilmiş tir. Bağırsak stomalı hastalarda stomanın cinsel yaşama olan etkisini ölçmekiçin cinsel yaşama spesifik altı farklı ölçek (Golombok-Rust Cinsel DoyumÖlçeği, Kadın Cinsel İşlev Endeksi, Uluslararası Erektil İşlev Endeksi, ArizonaCinsel Yaşantılar Ölçegi, Rektal Kanser Kadın Cinselliği Skoru, Cinsel VajinalDeğişiklikler Anketi) kullanılmıştır. Hastaların cinsel yaşam sorunları yaşadık ları ve erkeklerin kadınlara göre ostomi varlığından cinsel yaşamlarının dahafazla etkilediği bulunmuştur. Her iki cinsiyet için cinsellikten kaçınma, cinselsıklıkta azalma, cinsel ilişkiyi tamamlayamama, orgazm sorunları gibi sorunlarortak yaşanırken, kadınlarda cinsel ilişki sırasında ağrı, erkeklerde ereksiyonsorunlarının cinsiyete spesifik sorunlar olduğu belirlenmiştir. Hemşirelik mü dahalesinin hastaların cinsel yaşamlarına yeniden başlamada ve cinsel sorunla rında çözüme ulaşma da etkili olduğu saptanmıştır. SONUÇ: Bağırsak stoması, cinsel yaşamı olumsuz etkilemektedir. Bu siste matik çalışma, hastaların stoma ile ilgili cinsel yaşam sorunlarına ve cinselyaşamlarına adaptasyonlarına destek olabilecek hemşirelik müdahalesi çalış malarının olumlu etkisine rağmen, sınırlı sayıdaki literatüre dikkat çekmek tedir. Hastaların gereksinimleri doğrultusunda daha fazla araştırma yapılma sı gerekmektedir.

Öz:

OBJECTIVE: Intestinal stoma has a great impact on the individual, causingproblems in sexual life as well as various daily life-related difficulties. Thepurpose of this systematic review is to determine the sexual life problems ofindividuals with stoma. MATERIAL and METHODS: A systematic review was carried out basedon the searches of PubMed (MEDLINE), CINAHL, Cochrane Library,PsycINFO, Google Schoolar databases including the articles publishedbetween 2000–2020. For the study, articles reporting data from an originalstudy conducted using adult patients undergoing colostomy and ileostomysurgery and sexual life-specific scales were identified as inclusion criteria. RESULTS: A total of 10 articles with 7 descriptive, 1 prospective and 2experimental methods among the determined 1977 articles were includedin the study. 6 different scales specific to sexual life to measure the effectof stoma on sexual life in patients with intestinal stoma (Golombok-RustSexual Satisfaction Scale, Female Sexual Function Index, InternationalErectile Function Index, Arizona Sexual Experiences Scale, Rectal CancerFemale Sexuality Score, Sexual Vaginal Changes Questionnaire) used. It hasbeen found that the patients have sexual life problems and that the presenceof ostomy affects their sex lives more than women. While problems such asavoidance of sexuality, decrease in sexual frequency, inability to completesexual intercourse, orgasm problems were common for both sexes, it wasdetermined that pain during sexual intercourse in women and erectionproblems in men were gender-specific problems. It has been determinedthat the nursing intervention is effective in restarting the sexual life of thepatients and reaching a solution in their sexual problems. CONCLUSION: Intestinal stoma negatively affects sexual life. Thissystematic review draws attention to the limited number of literature,despite the positive impact of nursing intervention studies that may supportpatients’ sexual life problems related to stoma and their adaptation to theirsexual life. More research is required in line with the needs of patients.
