Antifosfolipid sendromunda kardiyovasküler risk: Trombotik ve obstetrik hastalık alt gruplarının karşılaştırılması

Antifosfolipid sendromunda kardiyovasküler risk: Trombotik ve obstetrik hastalık alt gruplarının karşılaştırılması
Öz: Amaç: Bu retrospektif çalışmada, Türkiye’de tek merkezden takip edilen primer (P) veya sistemik lupus eritematozusa (SLE) sekonder antifosfolipid sendromu (AFS) tanılı hasta kohortunda sadece vasküler trombozu (VT) olan ve sadece gebelik morbiditesi (GM) olan hasta gruplarının klinik ve laboratuvar özelliklerini karşılaştırarak tromboz gelişimi ile ilişkili faktörleri saptamayı amaçladık. Yöntem: AFS tanılı 169 hastanın (73 PAFS ve 96 SLE+AFS) demografik, klinik ve laboratuvar özellikleri İstanbul Tıp Fakültesi Romatoloji Bilim Dalı AFS veri tabanından elde edildi. IgG ve IgM izotipinde antikardiyolipin ve anti-β2-glikoprotein I (aβ2GPI) antikorları ELISA yöntemiyle, lupus antikoagulanı ise aPTT ve dRVVT testleri ile belirlendi. Hastalar kliniklerine göre sadece VT, sadece GM veya VT+GM gruplarına ayrıldı. Her hasta için düzeltilmiş global AFS skoru (aGAPSS) literatürde daha önce belirtildiği şekilde hesaplandı. Bulgular: Hastaların %47,3’ü sadece VT, %20,1’i sadece GM ve %32,5’i VT+GM olarak sınıflandırıldı. Elli dokuz hastada sadece arteriyel, 53 hastada sadece venöz, 21 hastada hem arteriyel hem venöz tromboz ve 2 hastada küçük damar trombozu vardı. Sadece VT ve sadece GM arasında antifosfolipid antikor (aFL) profili açısından farklılık saptanmazken, kardiyovasküler risk faktörlerinden hipertansiyon, hiperlipidemi ve sigara sadece VT grubunda daha sıktı (sırası ile %50,0 vs. 17,6, p=0,001; %45,0 vs. 23,5, p=0,02; %28,8 vs. 11,8, p=0,04). Sadece VT grubunda aGAPSS sadece GM’ye göre daha yüksekti (ortalama 9,2±3,8 vs. 7,3±2,9; p=0,01). Sadece arteriyel trombozu olanlarda sadece venöz trombozu olanlara göre daha yüksek hipertansiyon ve hiperlipidemi prevalansı saptandı (sırası ile %57,6 vs. %32,1, p=0,006; %54,2 vs. %32,1, p=0,015). Sonuç: Trombotik ve obstetrik AFS hastalık alt grupları arasında aFL profili açısından farklılık gözlenmezken, kardiyovasküler risk faktörleri artmış tromboz riski ile ilişkilidir. Kardiyovasküler riskin kontrol altına alınması AFS’de özellikle arteriyel tromboz riskini azaltabilir.
