A basic tool to determine exercise capacity in COPD: 4-meter gait speed
Öz: Introduction: Determination of exercise capacity is crucial for chronic obstructive
pulmonary disease (COPD) management since it is an indicator of life
quality and mortality. Various previously defined tests, including the cardiopulmonary
exercise test (CPET), the 6-minute walking test (6MWT), and the
incremental shuttle walk test, are used to determine COPD patients’ exercise
capacity. However, they all have some disadvantages, such as time, personnel,
and equipment requirements, limiting their daily practice use. Here, it was
aimed to investigate the role of 4-meter gait speed (4mGS) in evaluating exercise
capacity in COPD.
Materials and Methods: We included 40-to-70-year-old COPD patients with no
contraindications for exercise testing. We performed the tests at standard intervals
without affecting each other’s results. We also performed spirometry. We
used the modified Medical Research Council Scale, COPD Assessment Test, and
BODE index to determine the current status of individuals.
Results: Twenty-three (female/male= 2/21) subjects aged 58.1 ± 7.3 were
included in the study. A statistically significant correlation was observed between
4mGS and peak VO2 values. There was also a statistically significant relationship
between 4mGS and 6MWT distance. 4mGS correlated with CAT and
BODE scores to represent the general medical status of patients.
Conclusion: Considering its easy applicability and reproducibility, 4mGS
seems to be a strong candidate for daily clinical use in monitoring the exercise
capacity of COPD patients. Further studies in larger patient groups are
needed to support these results.

Öz: Giriş: Egzersiz kapasitesinin belirlenmesi, yaşam kalitesi ve mortalitenin bir göstergesi
olması nedeniyle kronik obstrüktif akciğer hastalığı (KOAH) yönetimi için çok önemlidir. Kronik obstrüktif akciğer hastalarının egzersiz kapasitesini belirlemek için kardiyopulmoner egzersiz testi (KPET), 6 dakikalık
yürüme testi (6DYT) ve artımlı mekik yürüme testi dahil olmak üzere önceden tanımlanmış çeşitli testler kullanılır. Ancak hepsinin
zaman, personel ve ekipman gereksinimleri gibi günlük pratik kullanımlarını sınırlayan bazı dezavantajları vardır. Bu çalışmada KOAH'ta
egzersiz kapasitesini değerlendirmede 4 metrelik yürüme hızının (4mYH) rolünü araştırmayı amaçladık.
Materyal ve Metod: Egzersiz testi için kontrendikasyonu olmayan 40-70 yaşındaki KOAH hastaları çalışmaya dahil edildi. Testler
birbirinin sonuçlarını etkilemeyecek şekilde standart aralıklarla gerçekleştirildi. Ayrıca her hastaya spirometri yapıldı. Bireylerin mevcut
durumunu belirlemek için değiştirilmiş modifed Medical Research Council dispne ölçeği, KOAH Değerlendirme Testi ve BODE endeksi
kullanıldı.
Bulgular: Yaşları 58.1 ± 7.3 olan 23 (kadın/erkek: 2/21) hasta çalışmaya dahil edildi. 4mYH ile tepe VO2 değerleri arasında istatistiksel
olarak anlamlı bir korelasyon gözlendi. 4mYH ve 6DYT mesafesi arasında da istatistiksel olarak anlamlı bir ilişki vardı. 4mYH ile
CAT ve BODE puanları arasında anlamlı korelasyon saptandı.
Sonuç: Kolay uygulanabilirliği ve tekrarlanabilirliği göz önüne alındığında, 4mYH, KOAH hastalarının egzersiz kapasitesinin izlenmesinde
günlük klinik kullanım için güçlü bir aday gibi görünmektedir. Bu sonuçları desteklemek için daha büyük hasta gruplarında daha fazla
çalışmaya ihtiyaç vardır.
