Cerrahi alan enfeksiyonları: risk faktörleri ve önleme yöntemleri

Öz:
Cerrahi alan enfeksiyonları mortaliteyi, morbiditeyi, hastanede yatış süresini ve hastane masraflarını arttırması bakımından cerrahinin çok önemli ve ciddi bir problemidir. Cerrahi girişimi takiben, eğer implant kullanılmamış ise 30 gün içinde, implant kullanılmış ise bir yıl içinde gözlenen, cerrahi insizyon ve açılan veya manipüle edilen alanla ilgili enfeksiyonlar cerrahi alan enfeksiyonu olarak adlandırılır. Bu enfeksiyonlar insizyonel ve organ/alan cerrahi alan enfeksiyonu olmak üzere iki gruba ayrılır. Ayrıca, insizyonel cerrahi alan enfeksiyonları da yüzeyel ve derin olmak üzere iki grup halinde incelenir. Bu enfeksiyonlar mikroorganizma ile hasta bağışıklık sistemi arasındaki dengenin enfeksiyon ajanı lehine bozulması sonucu gelişir. Cerrahi alan enfeksiyonuna neden olan mikroorganizmaların başlıca kaynağı cilt, müköz membran ve intestinal sistem endojen florasıdır. Beslenme bozukluğu, diyabet, sigara kullanımı, immün cevap yetersizliği, hastanede kalış süresinin uzaması, uygun olmayan antimikrobiyal profilaksi operasyon odasının yetersiz ventilasyonu, asepsi ve antisepsi tekniklerine uyulmaması, cerrahi alanda yabancı materyal bulunması ve cerrahi teknik gibi pek çok faktör cerrahi alan enfeksiyonu gelişiminde rol oynar. Öpere edilecek olan her hastanın bu risk faktörleri açısından değerlendirilmesi ve gerekli önlemlerin alınması cerrahi alan enfeksiyonlarının insidansını azaltacaktır. Bu çalışmada, cerrahi alan enfeksiyonları ile ilgili risk faktörleri ve önleme yöntemleri güncel literatür eşliğinde tartışılmaktadır.
