Adolesan gebeliklerde anne yaşının perinatal sonuçlara etkisi

Öz:

Amaç: Bu çalışma, adolesan dönem gebeliklerinde anne yaşının perinatal sonuçlar üzerine olan etkisini incelemek amacıyla yapıldı. Materyal ve Metot: Atatürk Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı’nda Ocak 2001 ve Aralık 2004 tarihleri arasında doğum yapan 376 adolesan gebe ve 400 kontrol hastası retrospektif olarak incelendi. Çalışma grubunu oluşturan adolesan gebeler 19 yaş ve altında, kontrol grubunu oluşturanlar ise 20-29 yaş arasında idi. Adolesan gebeler anne yaşına göre ²17, 18 ve 19 yaş grubu olmak üzere üç alt gruba ayrıldı. Sonuçlar: Adolesan gebelik insidansı %4.08 idi. Adolesan annelerin %88’inin ilk gebeliğiydi. Çalışma grubunun doğum sırasındaki ortalama gebelik haftası kontrol grubundan yaklaşık 2 hafta daha erkendi (p<0.001). Tüm gruplarda anemi, erken membran rüptürü, intrauterin gelişme geriliği, preeklampsi, konjenital anomali, ölü doğum ve perinatal ölüm sıklığı açısından istatistiksel olarak anlamlı bir fark yoktu. Vajinal doğum oranı adolesanlarda daha fazla olmakla birlikte bu oran istatisıtiksel olarak anlamlı değildi. Kontrol grubuna göre 18 yaş ve altındaki kadınlar daha yüksek erken doğum riskine sahipti (p<0.05). Adolesan hastaların üç alt grubunda da ortalama doğum ağırlığı kontrol grubuna göre daha az (p<0.001) ve düşük doğum ağırlığı (<2500 gr) oranı ise daha fazla idi (p<0.005). Ancak, iri bebek doğurma oranı benzerdi. Yaşı $leq$17 olan grupta eklampsi insidansı hem kontrol (p<0.001) hem de 19 yaş grubuna göre (p<0.01) daha fazla görüldü. Tartışma: Birçok perinatal sonuç açısından adolesan gebelerde risk artışı yoktur. Ancak, 18 yaşından küçük kadınlarda erken doğum riskinde, 17 yaş ve altındakilerde ise eklampsi gelişim riskinde artış mevcuttur.

Impact of maternal age on perinatal outcomes in adolescent pregnancies

Öz:

Objective: In this study, it was purposed to investigate the effects of maternal age of adolescent pregnancies on perinatal outcomes. Materials and Methods: This retrospective study, included 376 adolescent pregnancies and 400 controls delivered in Atatürk University, Department of Obstetrics and Gynecology, was conducted between January 2001 and December 2004. The study group consisted of adolescent pregnant women who were younger than 19 years old, and the control group who were at 20-29 years of age. Adolescent pregnancies were divided into three subgroups according to maternal age; ²17, 18 and 19 years. Results: The incidence of adolescent pregnancies was 4.08%. Eighty-eight percent of adolescent mothers were primiparous. Mean gestational week of study group at delivery was two weeks earlier than controls (p<0.001). There were not statistically significant differences between groups in frequency of anemia, premature rupture of membranes, intrauterine growth restriction, preeclampsia, congenital malformations, stillbirth, and perinatal mortality. Although vaginal birth rate was higher in adolescent it did not reach statistical significance. As compared with controls, women aged 18 years or younger were at higher risk of preterm delivery (p<0.05). The mean birth weight of three subgroups of adolescent patients’ were lower (p<0.001), and rate of low birth weight (<2500 g) was higher (p<0.005) compared to controls. But, prevalence of macrosomic infant was similar. The incidence of eclampsia was significantly higher in $leq$17 years old women group compared to controls (p<0.001) and to 19 years old group (p<0.01). Conclusion: Adolescent pregnancies have not increased risk of many perinatal outcomes. On the other hand, pregnant women aged 18 years or younger have a higher risk of preterm delivery and aged 17 years or younger have a higher risk of eclampsia compared to pregnant women at 20-29 years of age.
