Fibromiyalji tedavisi: rehabilitatif yaklaşım

Öz:
Fibromiyalji uzun süreli, yaygın ağrı ve sıklıkla yorgunluğun eşlik ettiği jeneralize hassasiyet ile karakterizedir. Fibromiyalji sendromlu hastaların tedavisinde sıklıkla kullanılan özellikle ilaç dışı rehabilitasyon yaklaşımlarının etkinliklerine yönelik çalışmalar ilaç tedavi etkinliğini değerlendiren araştırmalara göre daha azdır. Bu derleme içerisinde tedavinin vazgeçilmez bir bölümü olan rehabilitatif yaklaşımların konu edildiği çalışmalar içerisinde ağırlıklı olarak randomize, kontrollü olan araştırmalar gözden geçirilmiştir. Özellikle aerobik egzersizler, hasta eğitimi, kendi-kendine tedavi, ve bilişsel-davranışsal tedavilerin etkinliğinin diğer yöntemlere göre daha fazla olduğu söylenebilir. Tedavide yararlanılabilen başlıca egzersiz tipleri tüm vücut aerobik egzersizleri, bisiklet, dans, yürüme, havuz egzersizleri, germe ve güçlendirme egzersizleridir. İyi eğitimli, kendi kendine tedaviyi yürütebilecek ve semptom dalgalanmaları na göre düzenlemeler yapabilecek bir hasta, başarılı bir tedavi ekibinin en önemli üyesidir. En etkin eğitim programları sağlıklı davranışlara dayanarak öz-yeterliği vurgular ve öz-yeterliği daha fazla olan hastalar rehabilitasyon programlarına daha iyi uyum sağlamakta ve daha olumlu sonuçlar elde edebilmektedirler. Sonuç olarak fibromiyalji sendromunun birden fazla sisteme ait semptomlarla seyrettiği dikkate alınmalı ve birçok çalışmada da gösterildiği gibi yukarıda sayılan yöntemleri bir arada kullanan çok disiplinli (multimodal) tedavi yöntemlerini tercih etmek gerekmektedir

Fibromyalgia management: rehabilitation approaches

Öz:
Fibromyalgia is characterized by long-lasting, widespread pain and generalized tenderness, usually accompanied by fatigue. There are fewer studies on the efficacy of non-pharmacologic rehabilitative treatment approaches compared with pharmacologic treatment choices. In this review, particularly randomized controlled studies on rehabilitative techniques which are indispensable for fibromyalgia treatment have been overviewed. The efficacy of aerobic exercises, patient education, self management techniques and cognitive-behavioral therapies is more when compared with other treatment modalities. Main beneficial exercise types are whole body aerobic exercises, cycling, dance, walking, pool exercises, stretching and strengthening exercises. The most important member of a successful management team is a well-educated patient who has the ability to maintain the treatment program and make appropriate arrangements according to fluctuations in symptoms. The most efficient education programs emphasize self-efficacy and the patients who get beter results are those having more self-efficacy thus adapting more easily. Consequently, we must consider that fibromyalgia syndrome is characterized by multisystem symptoms and so multidisciplinary (multimodal) therapies should be preferred as it has been shown with many previous studies.
