The evaluation of 371 cases with cerebral palsy between january 1984 and december 2004

Öz:
Serebral palsi, postür ya da hareketin anormal kontrolü ile karakterize nonprogresif nöropatolojik bir durumdur. Santral sinir sistemi lezyonu, zedelenmesi veya mal formasyonuna sekonder gelişir. Bu çalışmada, hastanemiz Nöroloji Polikliniğinde Ocak 1984-Aralık 2004 yılları arasında izlenen ve serebral palsi tanısı alan 371 olgu geriye dönük olarak incelendi. Olguların 144’ü (%38,8) kız ve 227’si (%61,2) erkek; yaşları 7–216 ay arasında (ortalama 96,50 ± 40,09 ay) olarak belirlendi. Çalışmamızda spastik kuadriparezi %47,2, spastik diparezi %19,7 ve hemiparezi %15,6, mikst tip %9,1, hipotonik-ataksik tip %5,7, diskinetik tip %2,2 ve spastik monoparezik tipe %0,5 oranında rastlandı. En sık rastlanan etiyolojik nedenler, asfiktik doğum (%41,2), prematürite (%22,6), SSS infeksiyonları (%6,5) ve kernikterus (%4,6) idi. En sık rastlanan kranial görüntüleme bulguları %29,1 (n=108) ile periventriküler lökomalazi, %23,7 (n=88) ile hipoksik-iskemik değişiklikler ve %18,9 (n=70) ile diffüz serebral atrofi idi. Sonuç olarak, serebral palsi gelişiminde, önlenebilir nedenler olan asfiktik doğum ve prematürite halen en sık nedenleri oluşturmaktadır. Çocuk hekimi ile kadın doğum uzmanının işbirliği içinde çalışması, düzenli gebelik izlemi ve perinatal dönem patolojilerinin önlenmesi ile serebral palsi olasılığının azaltılabileceği kanısındayız

Öz:
Cerebral palsy is a nonprogressive neuropathological condition which is characterized by abnormal control of posture or motion. It develops secondary to a central nervous system lesion, injury or malformation. In this study, 371 cases which had been diagnosed as cerebral palsy and followed up by the neurology department of our hospital were retrospectively investigated. One hundred and fourtyfour (38.8%) of the cases were female and 227 (61.2%) were male; their age varied between 7–216 months (average 96.50 ± 40.09 months). The distribution of deficits was as follows: spastic quadriparesis 47.2%, spastic diparesis 19.7%, hemiparesis 15.6%, mixed type 9.1%, hypotonic-ataxic type 5.7%, dyskinetic type 2.2% and spastic monoparesic type 0.5%. The most frequent etiological reasons were asphyxia (41.2%), prematurity (22.6%), central nervous system infections (6.5%) and kernicterus (4.6%). The most frequent imaging findings were periventricular leukomalasia 29.1% (n=108), hypoxic-ischemic changes 23.7% (n=88) and diffuse cerebral atrophy 18.9% (n=70).
