Depremzedelerde ruh sağlığı hizmeti kullanımı: 1999 depremlerinin sonuçları

Öz:
Amaç: Ruhsal travmalardan sonra görülen ruhsal hastalıklar sağlık hizmeti alma oranlarını arttırsa bile, travmaya uğramış kişilerin büyük çoğunluğu yardım alamamaktadır. Bu çalışmada 1999 depremlerini yaşayan kişilerde sağlık hizmeti kullanımı oranları, başvurulan tedavici türü ve psikotrop ilaç kullanımının varlığı ile bu değişkenlerin yordayıcıları araştırılmaktadır.Yöntem: 17 Ağustos depremini yaşayan, merkezüstüne farklı uzaklıktaki iki merkezde (Değirmendere ve Avcılar), rastgele yöntemle seçilmiş 2007 kişi travmatik stres, depresyon ve sağlık hizmeti kullanımıyla ilgili anketler kullanılarak, depremden bir yıl sonra değerlendirilmişlerdir. Bulgular: Depremden sonra ruhsal bir sorun nedeniyle yardım arayanların oranı toplam grupta %13.7, herhangi bir sorun bildirenlerde %42 idi. Ruhsal sorun nedeniyle yardım aramayı belirleyen değişkenler arasında geçmiş ruhsal hastalık, depremden sonra depresif belirtiler yaşama ve maddi kayba uğramış olma vardı. Demografik değişkenler veya travmatik stres belirtileri ise sonucu etkilemiyordu. Sonuçlar: Tedavi ihtiyacı olan depremzedelerin bir kısmı bu yardımı alamıyor olabilir. Bulgular aynı zamanda afet-sonrası dönemde toplum taramalarının önemini göstermektedir.

Health services use for earthquake-related psychological problems: Results from the 1999 earthquakes in Turkey

Öz:
Objective: Although psychological disorders after traumas are associated with increased rates of services use, the majority of traumatized people do not receive psychological help. This study reports on the health services use of the survivors of the 1999 earthquake in Turkey. Method: A random sample of 2007 survivors was assessed in two sites, using self-report measures of traumatic stress, depression and use of health services.Results: Less than half (42%) of those with a post-earthquake psychological problem (13.7% of the total sample) reported contacting services after the earthquake. Health services use for psychological problems was predicted by past psychiatric illness, depressive complaints after the earthquake and material loss, but not by demographic variables or traumatic stress symptoms. Conclusion: The results show that earthquake survivors who need treatment might not be receiving it. They also show the importance of outreach programs in post-disaster circumstances.
