The role of endoscopic injection sclerotherapy in upper gastrointestinal bleeding due to peptic ulcers

Öz:

Peptik ülser kanamalarında endoskopik enjeksiyon uygulanması kolay, güvenilir ve etkili bir tedavi yöntemidir. Endoskopik enjiksiyon tedavisi, vazokonstriktör ve sklerozan maddelerin ülserin etrafına ve üzerine uygulanması esasına dayanmaktadır. 1994-1996 yılları arasında SSK Tepecik Hastanesi Gastroenteroloji Kliniğine, üst gastrointestinal sistem kanaması nedeniyle başvuran 44 hastaya endoskopik enjeksiyon tedavisi uygulandı. Yaşları 19-82 arasında olan hastaların 29'u erkek, 15'I kadındı. 42 olguda duodenal ülser, 2 olguda ise mide ülseri mevcuttu. Forrest sınıflamasına göre 5 olgu Ia (%11), 2 olgu Iia (%5), 20 olgu Ib (%45), 17 olgu Iib (%39) idi. 40 olguda (% 90.9) kanamanın kontrol altına alınmasına karşın, 4 olguda (%9) cerrahi tedaviye gereksinim duyuldu. 2 hastada oral antikoagülan (kumadin) kullanımına bağlı hemorajik diyatez söz konusu idi. Sonuç olarak peptik ülsere bağlı üst gastrointestinal sistem kanamalarında endoskopik enjeksiyon tedavisinin etkili bir tedavi yöntemi olduğu ortaya konuldu.

Öz:

Endoscopic injection sclerotherapy is a simple, reliable and efficient method of treatment for peptic ulcer bleeding. The principle of this method is based on the application of vasoconstrictor and sclerosing agents in and around the lesion. Fourty four patients with upper gastrointestinal bleeding hospitalized at the Gastroenterology Clinics of SSK Tepecik Educational Hospital between 1994-1996 were included in this study. The patients were aged between 19-82 years and the male/female ratio was 29/15. Fourty two cases were diagnosed as duodenal ulcer and two cases as gastric ulcer. According to Forrest classification, five cases were Ia (11%), two cases were IIa (5%), 20 cases were Ib (45%) and 17 cases were IIb (39%). In 40 cases (90.9%), bleeding were controlled by endoscopic injection sclerotheraphy while four cases required surgical treatment. Hemorrhagic diathesis was seen in two of the patients due to the use of coumadin. According to our results endoscopic injection sclerotherapy is an effective method of the treatment for upper gastrointestinal hemorrhage due to peptic ulcers.
