Güneydoğu Anadolu'da HCV'ye bağlı kronik hepatitlerde HCV genotipleri

Öz:

HCV'nun genotipleri ile karaciğer hastalığının klinik tablo ve interferon tedavisine yanıt arasındaki ilgi önemlidir. Bu amaçla, bu çalışmada bölgemizde ilk defa HCV'nun genotip tayini yapıldı. Çalışmamızda, anti-HCV'si pozitif, 50 kronik karaciğer vakasına ait serum örneklerini inceledik. Kadın/erkek hasta sayısı, 35/15 ve tüm hastalarda yaş ortalaması 39.6, yaş ortalaması kadınlarda 38.3, erkeklerde 41.0 idi. Hepatit C virus genotiplerini, Okamoto'nun tip spesifik primerlerin baz alındığı miksing metodu ile inceledik. Polimeraz zincir reaksiyonu kullanılarak 50 vakanın 28'inde HCV RNA pozitif, 22'sinde negatif bulundu. HCV RNA'sı pozitif olan 28 vakanın tümünde genotip 1b tespit edildi. HCV RNA'sı pozitif ve negatif vakalar arasında yaş, cins dağılımı, anamnez ve diğer özellikler bakımından fark yoktu. Sonuçlarımız, yurdumuzda yapılan diğer çalışmalarla tam uygunluk göstermektedir. Yurdumuzda değişik merkezlerde yapılan çalışmalarda büyük çoğunluk HCV genotip 1b tespit edilmiştir. Bu sonuçlara göre Türkiye'de, Güneydoğu dahil, HCV'na bağlı karaciğer hastalığı olanlarda hakim olan tip genotip 1b'dir. Çalışmamızdan çıkan bu sonuç, bölgemizde HCV'na bağlı kronik karaciğer hastalığının takip ve tedavisinde önemli bilgiler verebilir.

HCV genotypes in HCV related chronic hepatitis in Southeast Anatolia

Öz:

The relationship of HCV genotypes with the clinical course and prognosis, and their influence on response to interferon therapy is an important aspect of management of HCV related chronic liver disease. In this study, HCV genotypes were examined for the first time in our region. Serum samples of 50 chronic liver disease patients with HCV antibody positivitiy were analyzed. The male to female ratio was 35/15 with a mean age of 38.13 years (female) and 41 years (male). Hepatitis C virus genotypes were examined by means of Okamoto's mixing method which is based on type specific primers. Of the 50 patients, 28 were positive for HCV RNA and 22 were negative by polymerase chain reaction. We determined genotype 1b in all 28 patients who were positive for HCV RNA. There were no differences between HCV RNA's positive and negative patients in respect to age, gender, history and other findings. Our results are in correlation with those of other studies made in different regions of Turkey. According to these results, HCV genotype 1b is the predominant genotype in patients with HCV related disease in Turkey, including the Southeast region. The results of our study contribute important information to the management and treatment of HCV related chronic liver disease in our country.
