Eleştirel Sosyal Hizmet Çerçevesinde Şizofreni Tanılı Kişilerin ve Yakınlarının Damgalanma Deneyimleri: Zonguldak Örneği

Eleştirel Sosyal Hizmet Çerçevesinde Şizofreni Tanılı Kişilerin ve Yakınlarının Damgalanma Deneyimleri: Zonguldak Örneği
Öz: Bu araştırmanın amacı, şizofreni tanılı kişilerin ve yakınlarının damgalanma deneyimlerini ortaya koyarak eşitsizlik ve ayrımcılık pratiklerine görünürlük kazandırmaktır. Zonguldak Toplum Ruh Sağlığı Merkezi’nden hizmet alan kişiler bu çalışmanın popülasyonunu oluşturmuştur. Nitel araştırma yöntemiyle gerçekleştirilen bu araştırmada 17’si tanılı birey ve 15’i ise tanılı bireylerin yakınlarından oluşan 32 özne ile derinlemesine görüşmeler gerçekleştirilmiştir. Katılımcılarla yapılan derinlemesine görüşmelerden elde edilen veriler; sosyal ilişkilerde meydana gelen değişimler, öz-imajda meydana gelen değişimler, kamusal alanda eşitsiz koşullarla ve ayrımcılık pratikleriyle karşılaşmak ve bakım verenlere özgü deneyimler şeklinde dört ana tema çerçevesinde kavramsallaştırılmıştır. Araştırma bulgularından öznelerin tanı konulduktan sonraki süreçte yalnızlaştıkları ve kamusal alandan dışlandıkları anlaşılmıştır. Sosyal kimliğin damgalanma deneyimleri çerçevesinde inşa edilmesiyle birlikte özneler kendilerini toplumun gördüğü gibi görmeye başlamış, eksiklik ve mahcubiyet duyguları yoğunlaşmış ve tanı, kimliğin asli bileşeni hâline gelmiştir. Tanılı öznelerin çoğunun üretim ilişkilerine dâhil olamadığı tespit edilmiştir. Partnerleri tanılı olan kadınlar ise hem bakım rollerini üstlenmek hem de evin geçimini sağlamak gibi zorlu görevlerle karşı karşıya kalmışlardır. Bakım yükünü çoğunlukla aile üyelerinin üstlenmesi sonucunda bakım rolü gündelik hayat pratiklerinin önüne geçmiş ve özneler bakım dışında hiçbir şey yapamaz hâle gelmiştir. Ezcümle bu çalışma, damga mefhumunun yapısal ve ilişkisel veçhelerini ihtiva edecek şekilde kurgulanmış; böylelikle ruhsal sorunların toplumsal/politik arka planına ve tahakküm ilişkilerini sıradanlaştıran yasal düzleme ilişkin geniş kapsamlı bir çerçeve sunulmuştur. Buradan hareketle denilebilir ki, şizofreni tanılı kişileri ve yakınlarını bir araya getiren sivil toplum örgütlerinin eleştirel sosyal hizmet teorisinden dayanağını alan ve üyelerinin politikleşmesini sağlayan bir içerikle yeniden yapılandırılması gerekmektedir.
Anahtar Kelime: Stigmatization Experiences of Persons Diagnosed with Schizophrenia and Their Relatives in the Framework of Critical Social Work: The Example of Zonguldak
Öz: The aim of this research is to bring visibility to unequal social conditions and discrimination by revealing the stigmatization experiences of people diagnosed with schizophrenia and their relatives. Qualitative research design was used in this research, which was carried out with people who received service from Zonguldak Community Mental Health Center and in-depth interviews were conducted with 32 subjects, 17 of whom were diagnosed individuals and 15 of whom were relatives of diagnosed individuals. The data obtained from the in-depth interviews with the participants were conceptualized within the framework of four main themes: changes in social relations, changes in self-image, encountering unequal conditions and discrimination practices in the public sphere and experiences specific to caregivers. It was understood that the research subjects became lonely and excluded from the public sphere in the process after the diagnosis. As a result of the exclusion from social relations and the construction of social identity within the framework of stigmatization experiences, there has been a sharp decline in self-esteem. It was determined that most of the diagnosed subjects could not be involved in the relations of production. Women whose partners have been diagnosed have faced difficult tasks such as taking on care roles and providing for the household. As a result of mostly family members bearing the burden of care, subjects have become unable to do anything but care. In short, this study was designed to include the structural and relational aspects of the concept of stigma, and thus a comprehensive framework was presented regarding the social/political background of mental problems and the legal plane that trivializes relations of domination. Based on this, it can be said that non-governmental organizations that bring together people diagnosed with schizophrenia and their relatives need to be restructured with a content that is based on critical social work theory and ensures the politicization of its members.
