On dört erişkin viseral leyişmanyoz olgusunun değerlendirilmesi
Öz: Viseral leyişmanyoz ülkemizin de içinde bulunduğu Akdeniz ülkelerinde yaygın olarak görülen, tatarcık sineği (Phlebotomus) tarafından taşınan Leishmania protozoonunun neden olduğu hayatı tehdit eden sistemik bir enfeksiyon hastalığıdır. Bu çalışmada, Ocak 2005-Nisan 2012 tarihleri arasında kliniğimizde izlenen viseral leyişmanyoz olgularının retrospektif olarak irdelenmesi ve literatür ile karşılaştırılması amaç- lanmıştır. Çalışmaya, viseral leyişmanyoz tanısıyla izlenen ve tedavi süreleri tamamlandıktan sonra bir yıl boyunca takip edilen 14 olgu (7 kadın, 7 erkek; yaş aralığı: 19-64 yıl, yaş ortalaması: 41.6 ± 12.9 yıl) alınmış ve olgulara ait bilgiler hasta dosyalarından elde edilmiştir. Olguların 10’u immünokompetan, dördü immünosüpresif olup, altısı Adana’da sekizi ise güney ve güneydoğu Anadolu bölgesinin farklı illerinde ikamet etmektedir. Olgularda, belirtilerin başlangıcından tanı konuluncaya kadar geçen süre ortalama 75 (aralık: 2-272) gün olup, olguların %79 (11/14)’u daha önce bir sağlık kuruluşuna başvurmuş ve antibiyotik tedavisi almıştır. Olguların en çok ateş yüksekliği (%100), üşüme/titreme (%93), halsizlik (%71) ve kilo kaybı (%57) yakınmaları ile başvurdukları tespit edilmiştir. Fizik muayene bulguları irdelendiğinde; 8 (%57) olguda ateş, 11 (%79) olguda splenomegali ve 7 (%50) olguda hepatosplenomegali saptanmıştır. Olguların laboratuvar verileri değerlendirildiğinde; tümünde (%100) hipoalbünemi ve 10 (%71)’unda pansitopeni tespit edilmiştir. Viseral leyişmanyoz tanısı; 12 (%86) olguda kemik iliği aspirasyonu ve iki olguda doku (karaciğer/dalak) biyopsi örneklerinin yayma preparatında parazitin amastigot formunun görülmesiyle konulmuştur. Tüm hastalardan alınan kemik iliği örneklerinin NNN (Novy-MacNeal-Nicole) besiyerinde kültürü yapılmış, 4 (%29) olguya ait örnekte Leishmania promastigotları üretilmiştir. İmmüno- kompetan 10 hastaya 3 mg/kg/gün, immünosüpresif üç hastaya ise 4 mg/kg/gün parenteral lipozomal amfoterisin B tedavisi verilmiş, immünosüpresif bir hasta viseral leyişmanyoz tanısı konulduğu anda tedaviye başlanamadan kaybedilmiştir. Hastalığın prognozu irdelendiğinde, biri böbrek biri kemik iliği transp- lantlı iki immünosüpresif olgunun eks olduğu (mortalite oranı: %14), diğer olguların ise tamamen iyileştikleri tespit edilmiştir. Bu çalışma ülkemizden bildirilmiş, viseral leyişmanyoz ile ilgili en çok erişkin olgu sayısına sahip bir değerlendirme olup, hastalık özelliklerinin gözden geçirilmesini sağlamıştır. Sonuç olarak, endemik bölgede yaşayan ve ateş yüksekliği, hepatosplenomegali, pansitopeni ve hipoalbünemi saptanan hastaların viseral leyişmanyoz yönünden dikkatli değerlendirmeleri gerektiği kanısına varılmıştır.

Evaluation of fourteen adult cases with visceral Leishmaniasis
Öz: Visceral leishmaniasis is a life threatening systemic infectious disease caused by Leishmania protozoon, which is transmitted by phlebotomine sandflies, and is widespread in Mediterranean countries including Turkey. The aim of this study was to retrospectively evaluate the visceral leishmaniasis cases followed in our clinic between January 2005 to April 2012, under the light of the current literature. A total of 14 cases (7 female, 7 male; age range: 19-64 years, mean age: 41.6 ± 12.9 years) diagnosed as visceral leishmaniasis and followed for one year after their treatment were included in the study. Data of the cases were obtained from the patient files. Ten of the cases were immunocompetent and four were immunosuppressive. While six of the cases were residents in Adana, eight were from different cities of south and southeastern Anatolia, Turkey. The median period between the development of symptoms and diagnosis was 75 (range: 2-272) days and 79% (11/14) of them were admitted to a health center and used antimicrobial agents. The leading presenting complaint was fever (100%) followed by chills and shiver (93%), weakness (71%) and weight loss (57%). Physical examination revealed fever in 8 (57%), splenomegaly in 11 (79%) and hepatosplenomegaly in 7 (50%) cases. Based on laboratory findings, pancytopenia was detected in 10 (71.4%) and hypoalbuminemia was detected in all (100%) of the cases. The diagnosis of visceral leishmaniasis was made by the detection of amastigote form of the parasite in the smears of bone marrow aspiration for 12 (86%) cases and of tissue (liver/spleen) biopsies for two cases. Bone marrow samples obtained from all of the patients were inoculated into NNN (Novy-MacNeal-Nicole) media and only 4 (29%) of them yielded the growth of Leishmania promastigots. Parenteral lipozomal amphotericin B was initiated to 10 immunocompetent patients at a dose of 3 mg/kg/day and to three immunosuppressive patients at a dose of 4 mg/kg/day. One immunosuppressive patient lost his life before initiation of treatment. With regard to disease progression, it was detected that two cases (of whom one was renal and one was bone marrow transplant patient) lost their lives (mortality rate: 14%), however all the other cases were cured completely. The present study comprised the highest number of visceral leishmaniasis adult cases reported in Turkey and enabled the review of disease characteristics. In conclusion, cases presenting with fever, hepatosplenomegaly, pancytopenia and hypoalbuminemia and living in endemic regions should be precisely evaluated in terms of visceral leishmaniasis.
